Our care team encompasses Doctors, Advanced Practice professionals, Pharm D, Care Coach Nurses, Medical Assistants, Behavioral Health, Specialists, Quality-based Coders, and Referral Coordinators, ensuring a holistic approach and unmatched patient experience. In this role, you'll benefit from a supportive environment that allows you to focus on patient care, seeing fewer patients and spending quality time with each one. The total compensation package (base pay plus bonuses) may exceed $300K, based on experience. Key Responsibilities: Conduct evaluations and treatments for center patients in line with established care standards. Monitor care quality and outcomes using available data and chart reviews. Coordinate patient services, including specialty referrals, hospital and skilled nursing facility (SNF) coordination, durable medical equipment, and home health care. Participate actively in daily huddles with the care team, providing medical expertise and guidance. Collaborate with the Regional Medical Director and Center Administrator to foster a cooperative practice environment. Ensure timely completion of medical record documentation and collaborate with quality-based coders for optimized coding. Adhere strictly to policies and protocols set by Clinical Leadership. Engage with the Regional Medical Director regarding quality of care, outcome data, policy reviews, and medical records. Explore growth opportunities within the center for new or existing services. Participate in the local primary care on-call program as necessary. Maintain compliance with all licensing, certification, and accreditation requirements. Focus 100% of your time on clinical duties related to direct patient care. Desired Qualifications: Current and unrestricted medical license or willingness to obtain a Texas medical license. Graduation from an accredited MD or DO program. Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred. Active and unrestricted DEA license. Excellent communication skills, both verbal and written. Demonstrated proficiency in interpersonal relationships with colleagues and patients. Engagement in integrated team-based care models. Adaptability in practicing within a value-based care framework. Exceptional patient care and customer service skills. Basic computer literacy, including email and EMR systems. This role involves patient interaction and is part of our Tuberculosis screening program, requiring TB screening for selected candidates. Preferred Qualifications: Medicare and Medicaid Provider Numbers. Two to five years of relevant experience preferred. Experience managing a Medicare Advantage patient panel with knowledge of coordinated care best practices. Familiarity with Medicare guidelines and coverage. Understanding of HEDIS quality indicators. Bilingual in English and Spanish is an asset. Additional Information: Guaranteed base salary along with bi-annual bonuses.
