Physical Therapist (PT) - Contract -Springfield, OH Type: Skilled Nursing Facility / CCRC with outpatient services Duration: 8-week contract | 36 hours/week minimum Patient Age Range: 60-85 years Trauma Level: Short-term rehab (post-hospital stay) and long-term care Setting: Skilled nursing with CCRC and outpatient opportunities Responsibilities: Perform evaluations and establish treatment plans. Provide interventions in mobility, strengthening, gait training, balance, and functional activities. Accurately document care in accordance with facility and reimbursement guidelines. Collaborate with the interdisciplinary care team and educate staff/families. Maintain compliance with safety, infection control, and facility policies. Qualifications: Active (or eligible) Ohio PT license. Graduate of an accredited PT program. CPR/BLS preferred. Prior experience in SNF, rehab, or outpatient settings strongly preferred.
