For more information, visit This position will be fully remote for the first six (6) months and will then transition to an onsite role based in Washington, DC. Candidates must reside within the DC metro area, as this will be the permanent location of the pharmacy. Main Responsibilities· Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions· Maintains patient database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication· Ensures that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated and discussed with appropriate members of the healthcare team· Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and policy· Performs clinical intervention outreaches to patients, providers, caregivers, and other support personnel to ensure safe and appropriate patient therapy· Performs clinical intervention outreaches for identified opportunities to optimize patient therapy success, health and life outcomes, and patient experience· Performs clinical intervention outreaches to bridge care gaps as identified· Monitors, tracks, and provides reporting on patient outcomes as identified· Provides medication and disease state counseling to patients, caregivers, and other support staff· Monitors for and identifies medication related errors.
