Occupational Therapy Assistant Job Details Location: Santa Rosa, CA (serving North Santa Rosa, Windsor, Healdsburg, Geyserville, Cloverdale) Setting: Hospital-Based Home Health Employment Type: Travel Contract Schedule: Monday - Friday, 8:00 AM - 5:00 PM Guaranteed Hours: 40 hours per week Duration: 13 weeks (12/29/2025 - 3/29/2026) Shift: 8 Hour Days On-call/Weekend Requirements: None Productivity: 6 points per day EMR: HCHB (Homecare Homebase) Estimated Weekly Salary Range: $1,630 - $1,750 COTA Job Requirements Active Certified Occupational Therapy Assistant (COTA) license in California Minimum 1 year of experience in home health with a Medicare-certified healthcare facility Experience with HCHB EMR (Homecare Homebase) preferred Ability to work independently and as part of a multidisciplinary healthcare team Flexibility to assist outside primary territory as needed Strong communication and organizational skills Key Responsibilities for Occupational Therapy Assistants Deliver high-quality occupational therapy services to patients in their homes throughout the Santa Rosa, CA region Collaborate with occupational therapists and other healthcare professionals to develop and implement individualized care plans Document patient progress and maintain accurate clinical records using HCHB EMR Educate patients and families on therapeutic exercises, adaptive techniques, and home safety Meet productivity standards (6 points per day) and demonstrate flexibility by assisting outside the primary territory as needed Enjoy the vibrant arts scene, scenic vineyards, and beautiful Sonoma County while making a difference in patient lives.
