PRN Healthcare is seeking a local contract Physical Therapist for a local contract job in West Branch, Michigan. Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 03/16/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Essential Functions of Position: -Participates in the development of patient physical therapy plans of care. -Makes visits to the Patients for assessments and evaluations and communicates with the physician before start of care as needed. -Implements the plan of care. Documents the care and submits paperwork in an accurate and timely manner to update and maintain the Patient medical records. -Provides education and instruction to team members, Patients, and their families. -May participate in agency quality assessment performance improvement (QAPI) programs. -Accepts other assignments deemed appropriate. -Ensures the growth & profitability of the company through the responsible use of company resources and educating the community to our services. -Participates in regularly scheduled interdisciplinary team meetings to coordinate the care of the Patient and family, exchange information & problem solve & receive staff support and education. -Applies the policies and procedures of the agency and the rules and regulations of federal and state regulatory agencies and other certified agencies in providing care. -Provide skilled restorative treatments for pain reduction, develop/restore/maintain functions, including treatments using moist heat whirlpool (portable) paraffin baths, gait training, ultrasound electrical muscle stimulation, test and measurements for ROM (use of goniometer), instruct/establish/supervise exercise programs, prosthetic training, balance, coordination training, cardiovascular rehab, instruct in home safety. -Initial and ongoing comprehensive Assessment of the patient needs including outcome and information set (OASIS) assessment at appropriate time points. -Evaluating outcomes of care. -Directs and supervises Physical Therapist Assistant, Certified Nursing Aide, and Home Health Aide in care given to patients. Provides supervisory visits as necessary. -Willingly accepts direction from Director of Professional Services/patient Care Coordinator/Rehabilitation Supervisor. Schedule Notes: Monday-Friday 8a-5p. No call. 60-mile travel radius. HCHB required. Est. 6 patients/day. Required Skills & Experience: -Knowledge of principles, concepts, and methodology of physical therapy appropriate for terminally ill patients. Demonstrates problem solving and decision-making abilities. -Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention, and family intervention. -Excellent communication skills. -Ability to develop and maintain rapport with the patient, families, and staff. Preferred Skills & Experience: -One year or more work experience in physical therapy in a multidisciplinary setting is preferred.
