PrimaryOne Health is currently seeking a (Healthy Families of America) HFA Home Visit Supervisor for our Columbus health centers. Essential Responsibilities: Provides case management, care coordination, and patient education to ensure continuous and comprehensive care for patients on caseload in adherence to HFA and Growing Great Kids curriculum. Works collaboratively with the health care team including the providers, pharmacists, dieticians, behavioral health specialist, other nursing staff and medical assistants. Develop therapeutic relationships with women and their families in a home visiting environment in their caseload. Maintains communication with patients on caseload concerning treatment progress and diagnostic reports. Assess physical, emotional, social and environmental needs of women and their families as they relate to the HFA domain Completes documentation of all professional activities, contacts and observations promptly and appropriately. Attends internal and community meetings as assigned and participates in Quality Improvement activities. In-depth assessment and triage of more complex patients as identified by the provider group. Provide education, support and referral resources in assisting women and their families in attaining their targeted goals. Consult and collaborates with other professional involved in providing services to women and families. Participate in community coalition in support of HFA Staying informed of current health care developments within the field of maternal/child health, public health, and home visitation Support policies, procedures, guidelines and standards of HFA Assists in recruitment and enrollment of women and their families eligible for the HFA program Other duties or special projects as assigned. Regular travel required Successful candidates must have a bachelor's degree, strongly preferred in Social Work, Nursing, Early Childhood Educated, or related field; or less than a bachelor's degree but with 2-3 years of commensurate experience. 1-2 years' recent experience in a maternal/child health, public health, home visiting or equivalent combinations of education/experience preferred. Case management experience preferred.
