Join Our Caring Community and Prioritize Health The Patient Transition Specialist plays a crucial role in guiding patients through their healthcare journey, ensuring a smooth transition from hospital to home. This position focuses on both telephonic support and in-person engagement, helping hospitalized patients seamlessly adapt to their home environment after acute care. The Specialist serves as the vital link throughout the entire program, targeting specific quality measures and adhering to company protocols. This role requires traveling to hospitals for in-person visits with patients in various care settings, as well as conducting telephonic check-ins. Responsibilities include verifying patient demographics, reviewing equipment history, managing facility orders, and providing tailored support based on individual patient needs. You will be responsible for accurately documenting all interactions, including phone calls, interventions, and appointments within 24 hours. Daily management of tasks through our company system is essential for completing all assigned patient cases efficiently. Proactively identifying community resources through research and networking is also key in providing comprehensive support to patients. Maintaining hospital credentialing through the appropriate channels and ensuring compliance with all documentation is essential. Adherence to hospital conduct standards, policies, and procedures is expected, along with any additional duties assigned as needed. Make a Difference with Your Skills Preferred: Associate degree or higher from an accredited institution. At least 2 years of experience in customer service and healthcare in a fast-paced environment. Experience with Home Health and Durable Medical Equipment (DME) is highly desirable. Familiarity with medical terminology and the healthcare landscape is preferred. Proficiency in Microsoft Office applications, including Word, Excel, and PowerPoint. Additional Information On-site role Work Location: Memorial West Hospital (703 N Flamingo Rd, Pembroke Pines, FL 33026) Normal Work Hours: Monday to Friday, 8:00 AM to 5:00 PM (ET) On-Call Expectation: As needed Scheduled Weekly Hours 40 Compensation Range The estimated salary range for this full-time position (40 hours per week) reflects good faith efforts at the time of posting. Your actual pay may vary based on your specific skills, experience, and education, along with geographic location. The salary range is $59,300 - $80,900 per year.
