About the Job Position: Physical Therapist Assistant Department: Home Health & Hospice Supervisor: Clinical Manager of Therapy Services Job Summary: The Physical Therapist Assistant (PTA) delivers rehabilitative care under the supervision of a licensed Physical Therapist. The PTA provides treatment to home health and hospice patients to improve mobility, independence, safety, and quality of life. This role supports the agency's mission to deliver exceptional healthcare in the home and embodies the core values of Integrity, Respect, Teamwork, and Appreciation.Primary Responsibilities:Clinical Care:Implement components of the plan of care as directed by the supervising Physical Therapist. Provide interventions including ambulation training, transfers, gait training, and home exercise programs. Monitor and report patient progress, response to treatment, and changes in condition. Assess patient environment for safety and equipment needs; instruct patients and caregivers on proper use of durable medical equipment. Participate in interdisciplinary case conferences, care planning, and coordination with RNs, OTs, SLPs, MSWs, aides, and physicians. Document visits, clinical notes, and progress reports timely and in compliance with Medicare Conditions of Participation, PA licensure requirements, and agency policies. Resource Utilization & Compliance:Manage caseload and travel efficiently while maintaining high-quality, cost-effective care. Comply with all state, federal, and Medicare Home Health & Hospice regulations. Follow agency infection control, HIPAA, and patient safety standards. Seek guidance from supervising Physical Therapist and collaborate with the healthcare team as needed. Customer Service Excellence:Demonstrate respect, empathy, and professionalism in every patient and family interaction. Promote patient-centered care and maintain accountability for patient satisfaction throughout the episode of care. Provide education to patients, families, and caregivers regarding therapy goals and strategies for success. Performance Improvement & Collaboration:Participate in performance improvement initiatives, clinical record reviews, and committee work as assigned. Contribute to team problem-solving and support agency quality outcomes. Act as a positive representative of the agency in interactions with patients, families, staff, and the community. Secondary Responsibilities:Attend required staff meetings, trainings, and in-services. Support agency events, initiatives, and interdepartmental collaboration. Perform other related duties as assigned. Physical Requirements & Working Conditions:Visual/hearing ability sufficient for patient assessment and communication. Mobility to work in varied home and facility settings. Manual dexterity for documentation, equipment use, and communication tools. Ability to lift/carry up to 50 lbs occasionally. Current valid PA driver's license, automobile in good working condition, and proof of insurance. See attached Physical Requirements form. Qualifications:Graduate of a CAPTE-accredited Physical Therapist Assistant program. Licensed/certified as a Physical Therapist Assistant in Pennsylvania with certificate of authority to provide services under indirect PT supervision. Minimum of 2,000 hours of supervised experience; home health or hospice experience preferred. Current CPR certification.
