Medical Social Worker (CMSW)

Duties: Provide social work services to patients and their families in a medical setting Conduct assessments to determine the psycho-social needs of patients Develop and implement care plans based on patient needs and goals Collaborate with healthcare professionals to coordinate and facilitate patient care Assist patients and families in accessing community resources and support services Provide counseling and emotional support to patients and families Advocate for patients' rights and ensure they receive appropriate care Document patient interactions and maintain accurate records Supervise and document limited license medical social workers during counseling sessions Review and meet with LMSW to discuss their progress Experience: Master's degree in Social Work or related field Previous experience working as a Medical social worker preferred Knowledge of medical coding, research, and documentation practices undergraduate degree
