This role is an LPN who supports our Quality and Data Analytics team. Primary duties to include entering and preparing AWV-related orders in advance of patient visits, conducting outreach calls to patients, reviewing documentation for completeness for providers, and coordinating with practices for scheduling patients. This role works with all TMG practice providers, managers, and directors to ensure patient care gaps are addressed in a timely manner. Required Knowledge & Skills Education: Associate Degree or 2 years of college coursework Experience: One year of related experience. Requires a working knowledge of standard practices and procedures. Licenses and Certifications GEORGIA LICENSED PRACTICAL NURSE OR MULTISTATE NURSING LICENSE Qualifications Completion of a formal education program in practical nursing. Current state LPN licensure. Prior outpatient, quality, or population health experience strongly preferred. Experience with EMR's, data abstraction, patient outreach, or value-based care initiatives preferred. FunctionsArea of Responsibilities Entering and preparing AWV-related orders in advance of patient visits. Conducting outreach calls to patients to complete pre-AWV questionnaires. Reviewing documentation for completeness prior to scheduled AWV's. Coordinating with practices to ensure patients are appropriately scheduled and prepared. The LPN -- Quality, Data & Patient Outreach will perform data collection for the TMG Patient Centered Medical Home, Patient Centered Specialty Practice and Patient Center Connected Care. This information must be accurately compiled. This data will be collected from Epic electronic medical records and trended in spreadsheet formats to assist with the outcome measurement. Facilitation: The LPN -- Quality, Data & Patient Outreach works with the TMG quality team to close patient care gaps by utilizing payor portals to track progress. The LPN -- Quality, Data & Patient Outreach will assist rural health clinics in their annual program's evaluation chart reviews as well as immunization reimbursement reporting. The LPN -- Quality, Data & Patient Outreach will assist with Press Ganey NRC reports for leadership in order to improve patient satisfaction. Communication: The LPN -- Quality, Data & Patient Outreach will communicate effectively with leadership, internal and external clients. The LPN -- Quality, Data & Patient Outreach will have on-going communication with staff, medical staff, and leadership. Monitors program compliance with quality programs such as Patient Centered Medical Home and Rural Health requirements. Position may involve patient contact to coordinate needed care and close care gaps. Assist with the development of policies and procedures to ensure compliance with quality and regulatory standards. Serves as resource for various projects related to cost reports and other quality initiatives. Participates in Patient Centered Medical Home workgroups to prepare for certification and renewals. Works with leadership to prepare materials for workgroup meetings. Collaborates with management team and staff to identify and implement corrective action when needed. The LPN -- Quality, Data & Patient Outreach is a subject matter expert on requirements for Patient Centered Medical Home and other quality initiatives. Compliance Statement Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.
