About the Role: We're seeking a dedicated Nurse Practitioner to join our Salem, Oregon care team and is passionate about caring for medically complex seniors. This role blends clinic-based care, Telemedicine with occasional in-home visits approximately a 30 mile radius around the Salem area, allowing you to build meaningful relationships with patients while helping prevent unnecessary emergency department visits and hospital admissions. You will work closely with an interdisciplinary care team to address medical, functional, and psychosocial needs, particularly for patients with high-risk or chronic conditions, using a holistic and preventative approach. What You'll Do: Provide comprehensive clinical care for senior patients in clinic and home settings Conduct annual wellness visits, transitional care, and ongoing chronic disease management Support accurate documentation, Medicare risk adjustment, and condition capture Develop and execute individualized care plans with an interdisciplinary team Educate patients and families to support safe transitions and long-term health Maintain timely EMR documentation and participate in team collaboration Expected daily caseload: Consistent place of care each day Clinic: ~10-12 patients In-Home: ~5-6 patients Qualifications: Current and unrestricted Nurse Practitioner license in Oregon Board certification (FNP, AGNP, or related) through ANCC or AANP Minimum of 2 years' experience in adult/geriatric care, primary care, internal medicine, or related field Comfortable working with complex patients in home and outpatient environments Familiarity with Medicare populations, risk-based care, and HCC documentation Active DEA registration or willingness to obtain Valid Oregon driver's license CPR/BLS certification Home health or transitional care experience is a plus Benefits & Perks: Competitive Salary: $125,000-$150,000/year, commensurate with experience Up to $10,000 sign-on bonus or relocation assistance Competitive salary aligned with experience and market Comprehensive medical, dental, and vision coverage Paid time off, holidays, and parental leave 401(k) with employer match Professional development and continuing education support License and certification reimbursement Wellness resources and mental health support Malpractice insurance coverage Supportive care model with dedicated care coordination and physician leadership
