Role Responsibilities Deliver comprehensive care for individuals throughout their lives, from infants to seniors. Oversee chronic disease management, preventive measures, sudden illnesses, and mental health support with a patient-centric approach. Perform physical examinations, analyze diagnostic results, and craft individualized treatment strategies. Work alongside care coordinators, mental health experts, and other specialists for seamless, holistic care. Engage in initiatives aimed at uplifting patient outcomes and enhancing operational efficiency. Leverage electronic medical records (EMR) systems for detailed, current patient documentation. Provide both in-person and virtual care options based on patient preferences. Education Requirements MD or DO from an accredited institution. Completion of a recognized Family Medicine residency Certifications Board Certified in Family Medicine. Eligible for (or currently possessing) an active medical license in Minnesota. BLS and ACLS certifications are preferred. Skills and Experience Exceptional diagnostic and clinical abilities with a focus on proactive and preventive care. Comfortable working within value-based care systems and driven by a commitment to ongoing improvement. Outstanding communication, collaborative, and interpersonal skills.
