We have clinic locations in 16 counties in Georgia, including Appling, Atkinson, Bacon, Brantley, Bulloch, Candler, Charlton, Clinch, Coffee, Evans, Jeff Davis, Pierce, Tattnall, Toombs, Ware and Wayne counties. Job Responsibilities Under general supervision, will provide professional nursing care in accordance with nurse protocols and programmatic policies and procedures. Provides outreach, support and education to the target population and their surrounding community in order to reduce disparities in birth outcomes. Provides community health and case management services to high-risk perinatal women, infants, and children primarily through home visitation. Provides patient education and refers to agencies that can assist in meeting social and/or health needs. Records pertinent observations and response to treatment. Minimum Qualifications Associate's degree/diploma in nursing from an accredited college AND Completion of a PHN Course within one year of hire AND Current Georgia Registered Professional Nurse license AND CPR certification. Must have a valid driver's license and reliable transportation. Preferred Qualifications: Completion of a bachelor's degree in nursing. Bilingual-Spanish. Appropriate interventions for high-risk prenatal patients. Appropriate interventions for high-risk newborns and infants. Home visitation experience. One-year Labor and delivery experience, one year working in a prenatal care clinic, one-year experience working with mothers and babies.
