Join a dedicated healthcare team providing compassionate home health and hospice physical therapy services across the Midwest region. As a Physical Therapist in this role, you will develop personalized care plans following thorough patient evaluations, aiming to meet both patient and physician goals. You may work with home health or hospice patients and collaborate closely with interdisciplinary teams to ensure comprehensive care. Key Responsibilities Conduct patient evaluations for home health admission, adhering to CMS and payer guidelines. Perform timely, comprehensive assessments and develop individualized, age-appropriate care plans focused on self-management with measurable goals. Maintain ongoing communication with physicians regarding patient progress and any emerging issues. Document and implement physician orders promptly, updating care plans as needed. Engage patients and caregivers in care and discharge planning, tailoring education to their learning preferences and barriers. Manage resources efficiently, including scheduling visits and supplies. Supervise Physical Therapist Assistants (PTAs) and homecare assistants as assigned, ensuring alignment with care plans. Collaborate with multidisciplinary teams through meetings and direct communication to optimize patient outcomes. Assist patients in obtaining necessary equipment and initiate discharge planning from admission. Adhere to infection control and hand hygiene protocols during all treatments. Education Bachelor's degree in Physical Therapy from an accredited program recognized by the American Physical Therapy Association and American Medical Association is required. Experience At least one year of experience in an acute care hospital setting. Minimum one year in a rehabilitation facility or equivalent, serving adult populations. Preferred: Six months of home care experience. Computer proficiency is a plus. Certifications and Licenses Valid driver's license. American Heart Association Basic Life Support (BLS) certification. Current state license to practice physical therapy. Physical and Mental Requirements Ability to stand and walk for extended periods and navigate stairs. Strong communication skills, both oral and written. Excellent interpersonal and organizational abilities. Capability to safely operate a vehicle for 2-4 hours daily. Ability to maneuver around home environment obstacles and tolerate environmental factors such as temperature changes, smoke, fumes, dust, and pets. Working Conditions Frequent exposure to communicable and infectious diseases, potential contact with biohazardous materials, and varying community environments.
