Job Title: Occupational Therapist (Home Health - PRN) Location: Phoenix, AZ Employment Type: PRN / Per Diem (Flexible, rotating schedule based on patient needs) Work Setting: On-site, Home Health Salary: $88,000 - $105,000 annually ($42-$50/hour) Vendor Fee: 3% Job Summary: Seeking a compassionate Occupational Therapist to provide intermittent, home-based therapy services for senior patients. The OT will evaluate patients, develop individualized care plans, implement therapeutic interventions, monitor progress, educate patients/caregivers, and maintain accurate documentation. Key Responsibilities: Provide prescribed OT services to senior patients in their homes Conduct evaluations and develop treatment plans Implement therapeutic interventions to improve functional skills and independence Monitor patient progress and adjust care plans Communicate with physicians and care team Educate patients and caregivers on exercises/home programs Maintain compliant clinical documentation Collaborate with interdisciplinary teams Supervise therapy support staff as needed Required Qualifications: Graduate of an accredited OT program Current Arizona OT license Minimum 2 years clinical experience preferred Strong interpersonal, organizational, and documentation skills Preferred Qualifications: Home health care experience Proficiency in upper extremity rehab and ADLs
