Lead Care ManagerPay Rate: $22.00-$24.00/hr + mileage reimbursement Position Type:- 12-Month W2 Contract (renewal based on performance, project outcomes, and health plan continuation) Schedule: 9:00 AM - 5:30 PM PST, Monday-Friday Model: Field-Forward (multiple in-person visits weekly) Residency Requirement: Must currently live in county Star Nursing Services is hiring a Lead Care Manager to support CalAIM Enhanced Care Management (ECM) and Community Supports. This is a one-year contract with renewal consideration based on performance and program continuation. What You'll Do Lead Care Managers delivering Enhanced Care Management (ECM) and Community Supports Complete frequent in-person home visits, assessments, care plan updates, and transitions of care Support escalations and collaboration with health plan partners Train new staff on CalAIM workflows, county resources, and documentation Review EMR documentation for accuracy and MCP alignment Support audit readiness, findings, and CAP implementation Partner with ECM clinical teams, community agencies, and housing providers Important - Field Expectations Multiple in-person visits every week Daily travel across Santa Clara County Remote work is limited Required Qualifications Care/Case Management experience required Minimum 1 year in CalAIM ECM/CS, case management, public health, or related fields Strong understanding of Medi-Cal, Enhanced Care Management (ECM), Community Supports, authorizations, and managed care requirements Experience with appeals, grievances, audits, CAP findings, and corrective action plans Knowledge of County resources Strong communication and leadership skills 2 character references Must reside in Santa Clara County We are currently looking to place Lead Care Managers with experience in one or more of the following areas: Healthcare background Caregiver / CNA Social Work Medical Assistance Community Services Adult and Child Protection Experience in the following settings is a plus: RCFE (Residential Care Facilities for the Elderly) ALF (Assisted Living Facilities) SNF (Skilled Nursing Facilities) Home Health Hospice Behavioral Health / Psychiatric ICF / IDD Programs Veteran Homes Transportation Requirements • Unrestricted active California driver's license • Valid auto insurance • Current vehicle registration • Reliable vehicle for county-wide travel Technology Requirements • Intermediate experience with: - Microsoft SharePoint - Microsoft Excel - Microsoft Word - Microsoft Teams - Adobe (PDF editing, forms, e-sign tools) - EMRs for CalAIM documentation Minimum PC Specifications Windows 10 or higher Minimum 8GB RAM (16GB recommended) Intel i5 / AMD Ryzen 5 or higher 256GB SSD or larger Chromebooks not accepted Screenshot of computer specs required during interview Education AA/BA, or equivalent experience in case management, housing navigation, social services, public health, or related CalAIM functions Why This Role Matters Members face significant housing, financial, and health barriers.
