Are you an experienced Registered Nurse looking for meaningful one-on-one patient care-without sacrificing your work-life balance? At UR Medicine Home Care, you'll deliver skilled nursing care in the comfort of patients' homes while enjoying the flexibility and support you deserve. Our RN will administer skilled nursing care to patients in their place of residence. This is performed in accordance with physician orders and plan of care in conjunction with the Registered Nurse Case Manager and under the direction and supervision of Clinical Team Manager. The registered nurse executes the plan of care as established by the Registered Nurse Case Manager and makes necessary revisions as needed, furnishes those services requiring substantial and specialized nursing skill, provides appropriate preventive and rehabilitative nursing procedures, prepares clinical and progress notes, informs the physician and other personnel of changes in the patient's condition and needs, counsels the patient and family in meeting nursing and related needs, participates in in-service programs, and supervises and teaches other nursing personnel. Minimum Qualifications: New York State Licensed RN. One year of experience as a clinical nurse in a hospital required 9 months recent experience in home care required. Current NYS driver's license Responsibilities: Develops and revises plan of treatments and discharge plan in conjunction with patient, family, physician, care delivery team and third party payors including obtaining medical orders and approval of medical services. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames. Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries. Provides those services requiring substantial specialized nursing skills. Provides supervision of LPN to ensure patient health, safety, and compliance with plan of care. Maintains productivity requirements. Ensures that appropriate visit authorizations are in place and that visit documentation reflects the medical necessity for care to be received in the home setting.
