Job Description

Full Time (80 Hours/Biweekly)Shift: DaysMonday - FridayGeneral SummaryScreens, assesses, and monitors nutrition status of patients. As a member of the intradisciplinary care team, collaborates to identify patient needs and barriers to care. Evaluates nutrition needs as they relate to symptoms, therapeutic interventions, and mutually determined treatment goals.Essential Functions: Performs nutritional screenings, assessments, and follow up care involving review of patient charts, diagnoses, and nutritional history. Develops and implements a nutrition care plan, as appropriate. Documents all relevant aspects of nutritional care for patients in accordance with institutional and departmental standards. Reviews, updates, and coordinates patient nutritional care through ongoing communications with nursing, providers and other members of the health care team. Provides patients with nutrition guidance and education based upon their goals, current and past medical history, social environment, and behavioral health needs. Develops and implements a nutrition education plan. Teaches group nutrition classes. Draws on knowledge & experience of conventional medicine as well as integrative and functional medicine to holistically drive nutrition care. Represents clinical nutrition at multidisciplinary committees, care planning meetings, task forces, and project groups. Participates in performance improvement initiatives. Provides appropriate training to students, interns and new employees. Common Expectations: Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s). Maintains appropriate records, reports, and files as required. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, precepting dietetic interns, and workshops. Maintains required continuing education requirements as mandated for maintenance of registration and license. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation. Completes annual clinical competencies and facility-based mandatory training. Serves as an educational resource to peers, allied health and medical professionals. QualificationsMinimum Education: Bachelors Degree In Food and Nutrition or related field. Required Work Experience: 1 year Knowledge of foods/nutrition, food preparation, production and supervisory/instructional techniques. Required Experience in nutrition. Preferred Licenses: Registered Dietitian / Registered Dietitian Nutritionist Upon Hire Required or Certified Nutrition Specialist Upon Hire Required and Licensed Dietitian-Nutritionist within 180 days Required Courses and Training: Completion of approved Accreditation Council for Education in Nutrition and Dietetics ACEND accredited, supervised practice program. Upon Hire Required Nutrition Support Dietitian certification (CNSD).
