Summary: The Arc of Union County Inc. is a leading provider of Person Centered services to individuals with intellectual and developmental disabilities through Adult Day Services programming. Under the guidance of Assistant Executive Director, Clinical Services or designee, the Director, Day Behavioral Health is responsible for overseeing the provision of evidence-based Behavioral Services to adults with developmental disabilities, mental health disorders and other comorbid conditions. The role encompasses a comprehensive assessment of the behavior(s)/triggers, collection and review of data, development of structured behavioral modification plans and utilization of evidenced based practice, implementation of the plans, ongoing training and periodic reassessment of the plans consistent with BACB standards. The Director, Day Behavioral Health has full administrative responsibility and accountability for the quality of day habilitation program and behavioral services delivered in the Behavioral and Behavioral Health Program. The Behavior and Behavioral Health Day Habilitation Program is responsible for providing hands-on evidence-based Behavioral intervention services for improving challenging behaviors to adults with developmental disabilities and other associated co morbidities. This position will assure the effective implementation of "person focused" plans for all service recipients. The incumbent will actively implement developed program systems, policies and initiatives to meet the identified needs of our individuals served and families and provide recommendations to the Assistant Executive Director, Clinical Services for review. The incumbent will oversee compliance with program policies and procedures, and ensure that the programs operate in a safe, therapeutic and efficient manner. This position is responsible to ensure that all programming provides education and training to acquire the skills and experience needed to participate in the community, consistent with the participant's Service Plan. This includes activities that support participants with building problem-solving skills, self-help, social skills, adaptive skills, daily living skills, and leisure skills. Activities and environments are designed to foster the acquisition of skills, building positive social behavior and interpersonal competence, greater independence and personal choice. The Behavior and Behavioral Health Day Habilitation Program ensures that the service recipients are engaged in small group outings to the community, leisure, educational activities and community experiences. The incumbent is responsible for the implementation of Behavioral Support Activities that includes Behavioral assessments, Development of behavior support plans, Implementation of behavior plans, Periodic approval and reassessment and monitoring of behavioral plans consistent with the Arc of Union County's DDD approved Behavior Support Manual/Division Circular #34. The incumbent ensures that behavioral needs are identified, and measurable outcomes are achieved through participation in Behavioral intervention and are included in the Individual Service Plan (ISP). This position will oversee the implementation of a philosophy that enhances achievement of service recipients' full social inclusion, independence, and personal and economic well-being.The incumbent provides oversight to ensure implementation of strategies necessary to assist the individual in progressing toward his/her personally defined outcomes as indicated in the Individual Service Plan (ISP). This position requires thorough knowledge of Fee for Service model system. The incumbent is a member of a support team that assesses individual support needs and determining relative need for services. The Director, Day Behavioral Health will assure a high level of quality program supervision and management. The incumbent will assemble staffing teams and directly Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral and other personnel as assigned or required. This position is responsible for staff evaluations and personnel actions. The incumbent will provide oversight in the assembly of staffing teams and assure the development of staff competencies. Foster and maintain positive employee/management relations; develop an atmosphere of cooperation and teamwork. The Director, Day Behavioral Health develops high quality Behavior Services within the program and oversees the delivery of behavior services and supports. The Director, Day Behavioral Health will assure that the Behavior Program maintained in acceptable practices and appropriate conditions at all times for 3 Year Certification (Day Habilitation Certification), City Fire Safety Certification and full accreditation by CARF, etc. This position requires completion of DDD System Mandatory Training Bundle within 90 days of hire, DDD and Agency Orientations, other designated trainings and a minimum of 12 Hours of Professional Development completed annually. This position requires that information related to service recipient, employee, program operations is handled, channeled, managed and delivered professionally to the appropriate and right personnel or party consistent with agency and department policy. This is an exempt position and the incumbent reports to Assistant Executive Director, Clinical Services. Skills: This position requires Proficiency in Windows software, Ability to use a data entry software such as Therap, Ability to complete Shift and Individual Served ISP Data entry in web based electronic platformExperience writing and implementing Functional Behavior Assessments, Behavior Support Plans, Descriptive Assessments as well as documenting and analyzing behavior.Demonstrate active listening. Ability to exercise evidence based clinical decisions. This position requires excellent interpersonal and conceptual skills. Must be Flexible. Demonstrate ability to meet CARF accreditation and Day Habilitation Certification requirements. Qualifications: Minimum six years experience working with individuals with developmental disabilities and mental illness, minimum 5 years of successful frontline to upper management supervisory experience, programmatic operations and administrative experience required. Demonstration of a thorough understanding of knowledge, abilities and skills outlined above. Board Certified Behavior Analyst - Doctoral (BCBA-D)-OR- Board Certified Behavior Analyst (BCBA) -OR- Masters-level Clinician -OR- PHD-level Behaviorist -OR- Clinician holding NADD certification. Eight Hour Supervisor Training Course Certification for BCBA Supervision required. Must have a thorough knowledge of all DDD regulations. Must be at least 21 years of age and be of good moral character. Must have a valid New Jersey driver's license and abstract (not to exceed 5 points), and ability to meet agency insurability requirements. Major Position Responsibilities Develop operational plan for the Associate Executive Director Operation's review including but not limited to strategies for increasing referral volume, increasing service capacity, manage existing accounts, fulfilling service contract objectives, driving sales, productivity and overall business growth. Facilitate new business developments as directed, market new and innovative ideas. Design and ensure implementation of an Annual Program Schedule for the Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral to enhance delivery of varied therapeutic enhancement activities. Oversee, facilitate and monitor the delivery of behavior services including functional behavioral assessments, behavior support plans, behavioral strategies, behavioral interventions, behavioral training, etc. Assure all required information and documentations for service recipient's placement are obtained and filed for effective plan development, effective service delivery, acquisition of a 3 Year Day Habilitation Certification, fulfillment of Center for Medicaid Services requirements and agency objectives. Implement and ensure that behavioral Supports Practices are in compliance with the Arc of Union County's DDD approved Behavior Support Manual/Division Circular #34, licensing and other state regulations. Serve as a backup for the Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral. Ensure that service recipients have been assigned appropriate behavioral plans and supports consistent with their needs. Asses implementations, provide feedback on existing behavioral strategies, plans etc., and to assure that all behavioral needs are captured/documented and appropriate interventions and supports provided to staff in a timely manner. Vest time to observe and assure overall environmental assessment, comprehensive assessment, assessment of staff skills and designing/facilitating trainings, attain general awareness and understanding of what is effective versus non-effective, etc. Allow self to know what behaviors are active and not being documented or tracked, etc. Audit behavioral entries in Therap and provide training and guidance on clinical documentation as needed. Regularly supervise and guide the creation of evidence-based individualized Behavioral Support Plans and clinical reports in Therap. Follow through promptly with staff training on Antecedent Behavior Consequence (ABC) tracking, behavioral and environmental strategies, redirections, etc. Assure that all identified behavioral needs are met and services are delivered through coordination of service delivery. Train staff on Individual Behavior Support Plans (IBSPs) and implementation of ABA. Deliver trainings to all staff to enhance their understanding of the principles of Applied Behavior Analysis consistent with BACB standards. Develop and implement evidence-based tools such as competency testing aimed at preparing staff to successfully attain professional certifications such as BCBA, etc., consistent with BACB scope of practice and standards. Utilize Applied Behavior Analysis for behavioral management and environmental assessments. Formulate, write, and revise behavior support plans as needed, in accordance with functional assessment findings. Conduct annual review meetings of Behavior Support Plans for all service recipients. Design and oversee applied behavior analysis (ABA) implementation for service recipients. Provide direct supervision of Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral. Oversee functional assessments of new or increasing behaviors for service recipients as needed. Complete Behavioral Assessments, Implementation of Behaviors on the ISPs, monitoring of plans and assuring completion of case notes consistently. Notify the Associate Executive Director, Operations immediately of any situation, which compromises the safety and effective operation of Adult Services. Attend and participate in the Human Rights Committee and the PCT committee as assigned. Attend and participate in the Behavior Support committee. Maintain the Behavior Management Manual in accordance with Division Circular #34. Ensure documentation reflects that any approved rights restrictions, Behavior Support Plans (BSP)/guidelines have been reviewed within the previous year and that all required approvals from HRC, BSC, Planning Teams, physicians and guardians were obtained consistent with 10:44A - 3.1(a) & Subchapter 4.3 & 10:44C - subchapters 3.6 & 4.2). Schedule IDT Meetings to review and approve Individual Behavior Support Plan prior to implementation or inclusion into ISP. Attend and participate in meetings pertaining to service recipients. This will include but not be limited to ISP, IDT, and pre-placement meetings. Work collaboratively with residential directors on new admission intakes as recommended by Residential Admissions Committee and follow through on behavioral information for prospective candidates. Coordinate with healthcare support team on any therapeutic services needed or recommended such as Cognitive Behavioral Therapy, etc. Ensure all documented behaviors on the service care needs are captured in ABS/ISP. Oversee completion of Behavioral Sections of ISP and ABS/HSRS. Develop appropriate behavioral plans and strategies for all service recipients that meet BACB standards and utilization of evidenced based practice. Ensure review of behavioral information for a newly admitted person/readmitted person. Ensure that behavioral needs information, behavioral data, analysis, strategies, and Behavior Plans are entered in agency network platform (Therap system) for tracking and to enhance collective team reviews, effective audits/oversight and instant access to data by various disciplines. Review data consistent with BACB standards and scope of practice. Ensure that behavioral reports intended for psychiatrists are forwarded to Associate Executive Director, Operations for review and input. Submit verbal, written and graphic communication to Associate Executive Director, Operations review. Provide guidance to Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral to develop and enhance their understanding and skills required for appropriate behavioral supports practices and procedures including documentation of behavioral status of individuals served and in-house behavioral related procedures. Assess service recipients' on arising behavioral needs and make behavioral recommendations as soon as possible. Assess and identify the behavioral needs of the Behavior Program service recipients on an ongoing basis and make recommendations for further examination or professional consultation/intervention as necessary to provide appropriate responses to meet those needs within the guidelines of the agency. Collaborate with support team to ensure effective communications to enhance the quality of services and meet the service recipients' behavioral objectives and goals. Provide oversight in service delivery to individuals with deficits in the following areas: Sensory/motor, cognitive abilities, communication, social interaction and sociability, self-direction, self- care/independent living skills, behavioral concerns, health, employment and be part of a team that determines relative need for services and supports. Secure and provide oversight in planning for Prevocational Activities including, but are not limited to: job clubs, employment related skill classes, communication development, workplace safety, volunteer experiences (job sampling). Collaborate with management Workforce Development, Employment services to enhance the Prevocational Program. Provide oversight to ensure service recipients are supported and assisted in educational, enrichment or recreational activities as outlined in his/her Service Plan including community involvement. Plan and assure that each service recipient participates in out of program community outing and/or experiences at least three times a week as feasible. Provide oversight in service delivery and determine ongoing needs for services and supports are reviewed and revised as needs change. Ensure service recipients receive age-appropriate education and training to acquire the skills and experience needed to participate in the community, consistent with the participant's Individual Service Plan. This may include activities to support participants with building problem-solving self-help, social, adaptive, daily living, and leisure skills. Activities and environments are designed to foster the acquisition of skills, building positive social behavior, interpersonal competence, greater independence, and personal choice. Assure programming that promotes individual interactions and experiences by emphasizing on age-appropriate community experiences, and offering a variety of activities and choices. Oversee that planning, supports and assistance are provided to service recipients as outlined in their Service Plans to enhance inclusion in the community. Oversee planning of service recipients' indoor and group outings to community events. Assure the implementation of comprehensive, programming and service delivery systems are in accordance with Supports Program Standards, Communicate care program standards, Day Habilitation requirements, DDD, department/agency expectations, and CARF accreditation standards. Assure planning, supports and assistance are provided to service recipients in educational, enrichment or recreational activities as outlined on their Service Plans to enhance inclusion in the community. Assure planning of service recipients' indoor and group outings to community events, leisure and educational activities in the community. Plan and assure that each service recipients participate in out of program community outing and/or experiences as deemed appropriate. Design Annual Activity Schedule for Programs with varied discipline lessons for review and approval by department head. Oversee that the implementation of behavioral supports to service recipients as outlined in their Service Plans to enhance inclusion in the community. Oversee the assessment of behavioral therapeutic elements for individuals served and consult with Behavior Day Habilitation Program support team as needed for an appropriate resolution i.e. lack of motivation in programs, inability to socialize, etc. Provide supervision, coaching, mentoring, training and direction to Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral. Promote teamwork and trust through effective leadership and role modeling. Provide management oversight to Behavioral and Behavioral Health Day Program. Design trainings for Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral with increased focus on behavior and prevention of elopement. Ensure that staff receives high quality customized behavioral skills trainings that are relevant to the behavioral needs of the service recipients. Assure staff development resources and that opportunities are provided to all assigned staff as indicated. Develop proposals for program enhancement for submission to the Associate Executive Director, Operations for review. Assure Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, are facilitating and providing direction on agency/departmental new employee on-boarding communications, expectations and administrative orientations. Oversee the assembly of a successful management team and frontline staffing team. Assure completion of Personnel Status Notice in a timely manner upon notification of staff status change or execution of approved status change; submission to next level supervisor consistent with protocol for review and further processing. Assure completion and submission of Personnel Requisition as necessary in a timely manner for prompt position posting and hiring of staff. Employ, train and maintain a list of at least 5 Day Behavioral Supports and Behavioral Health Support Professional Substitutes. Assess Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral performance on a regular basis and ensure full implementation of departmental and agency goals. Assure the implementation of comprehensive community integration, recreation, programming and service delivery systems as designed and in accordance with Supports Program Standards, Day Habilitation requirements, DDD, department/agency expectations, and CARF accreditation standards. Assure processing of donation requests annually on behalf of individuals served for invitation to Courier News Wish Book for the Benefit of Individuals served. Monitor and assure compliance with all personnel and program policies/procedures. Provide guidance and assure consistency on agency progressive discipline policy. Seek guidance from immediate supervisor on managerial and supervisory matters for desired administrative outcomes. Assure required administrative and HR authorizations prior to executing personnel actions. Assure administrative review of letters, correspondences etc. intended for internal and external distribution. Oversee the development and monitoring of staff schedules and assure the appropriate coverage required to provide the highest quality of service delivery. Regularly audit operational staff schedules and assure alignment with department/administration/HR/budget and program description records. Ensure that staff audit and/or configure Medication Administration Records (MARs) in a timely manner, comparing MAR to prescription and Bubble pack to assure accuracy. Assure ongoing monitoring and documentation of medication administration, medication counts, and reordering as well as service recipients health needs on agency documentation platform (Therap). Identify any emerging health needs and notify appropriate personnel. Assure adequate medication refills, adequate medication supply at all times and proper medication administration standards. Assure staff members are writing the date, the time and their initials on the bubble pack/bingo cards after administering medication. Conduct meticulous random medication pack/bingo cards audits to ascertain proper medication administration and charting. Assure programs have adequate Personal Protective Equipments, cleaning supplies, etc. Flexible to support staff, program, department or agency in completing outstanding projects, tasks or operational needs and serve on agency committees as assigned. Audit and maintain policies, procedures and systems which involve service recipients, agency funds and agency property, use of agency and contract resources applicable to assigned programs and assure compliance with all DHS, Day Habilitation Certification standards, governmental and agency regulations and requirements. Ensure routinely agency property and funds are safeguarded. Responsible for obtaining from Finance/Accounts Payable monthly or frequently a print out of a list of the service recipients' recorded checks issued to the supervisors, audit the service recipients' electronic ledgers and account for accurate and timely entries of the checks received/associated cash entered in each service recipient's Therap electronic ledger. Assure the development, maintenance and accounting of all financial records and reports for program and service recipients' finances. Ensure effective management of agency petty cash and service recipients' funds, proper documentations appropriate practices consistent with agency policy. Assure financial report is submitted on time. Monitor budget uses and assure resources are utilized within the framework of the approved budget. Recommend budget modifications as required to assure effective and efficient service delivery. Generate and reconcile requisitions, audit and validate expenses on receipts/invoices. Execute the agency procurement policies, procedures and duties. Audit utilization of budgeted staffing hours and assure compliance. Responsible for the operation, quality, integrity and financial success of the Behavior Day Habilitation Program. Assist in the development and management of financial and operating budgets in the Behavior Day Habilitation Program. Foster and maintain positive employee/management relations and develop an atmosphere of cooperation and teamwork. Responsible for scheduling and facilitating Monthly Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral Meetings to provide training, assure a productive flow of communications and to also address service recipients' and programmatic related issues. Establish and implement appropriate management controls to assure effective and efficient use of agency resources including staff, finance, and equipments in compliance with agency budgets. Provide oversight on staff's time sheets review and submission to payroll for processing. Monitor and ensure that staff is following assigned schedules as well as clocking in/out payroll procedures. Review, approve and submit to payroll system time sheets for staff. Provide oversight on auditing and validation of expenses on receipts/invoices. Review monthly "Expense Report", correlate with requisitions processed/associated Records of Expense. Obtain a print out of a list of the service recipients' recorded checks issued to the Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, from Finance/Accounts Payable, audit the service recipients' electronic ledgers and account for accurate and timely entries of the checks received/associated cash entered in each service recipient's Therap electronic ledger. Assure the development, maintenance and accounting for all financial records and reports for program and service recipients' finances. Ensure effective management of Behavior Day Habilitation Program petty cash and service recipients' funds, proper documentations and appropriate practices consistent with agency policy. Assure financial report is submitted on a timely manner. Inform Associate Executive Director, Operations about any potential operational problems or issues. Assure that the Behavior Day Habilitation Program site and equipments are maintained in a manner providing safety and comfort to all individuals served and staff members. Routinely obtain staff input/feedback, interact with staff/service recipients and observe service delivery relative to operational objectives. Assure the presence of self or Assistant Direct presence in the morning to observe the arrival of service recipients, oversee and direct the supervision and smooth flow of service recipients into Service Delivery Rooms, lead in the organization, startup and participation the day's activities. Assure that the Assistant Director spends adequate time in the Service delivery Rooms evaluate the quality of the service delivered, provide guidance and feedback to supervisors and staff on the floor. Assess, evaluate and review programmatic practices. Observe and assess Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral on service delivery methods, and practices in conformance with department objectives. Provide guidance, support and mentoring as needed. Visit Service Delivery Rooms, observe and assure that supervisors as part of the direct support-staffing ratio, spends time on the activity floor, lead in facilitating programming activities, and provide self in supporting and relieving staff during shift sessions as well as asses, inspect the environment. Assure the Behavior Day Habilitation Program attains an overall performance rating of 88% or higher in internal and external expectation, objective, conformity audits/reviews. Assure Behavior Day Habilitation Program operational practices align with DDD Day Habilitation Standards, Supports Program Standards, and Agency Standards etc. Ensure the programs are fully prepared for a successful day habilitation inspection/audit and attainment of a 3 Year Day Habilitation Certification or the highest level of certification/accreditation. Design a training plan for the development Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral. Market Behavior Day Habilitation Program to the community and other external relevant parties to build a referral base and garner potential placement referrals as directed. Assure each service recipient has a current Individual Service Plan (ISP), a Person Centered Plan (PCPT) or Service Detail Report (SDR). Review provided information related to the individual, including the Person Centered Planning Tool (PCPT), previous ISP(s), available assessments, and evaluation data, as appropriate/relevant. Participate actively in the planning of team meetings and work cooperatively to achieve consensus in the spirit of the ISP operating principles. Participate in the IDT, ISP process and assure identified goals are accomplished in accordance with Agency policies. Assure correct implementation of ISP objectives and development of Behavior Plans as needed. Oversee implementation of skill development, Behavior plans, ISP's and other plans as developed and directed. Ensure that Therap activity report is run daily, auditing and verifying that each staff logs into Therap Daily and reviews all logs, email, & SComm as required, each staff rotates daily in attending to or supporting different individuals in program, each staff rotate in various shift tasks namely: Rotation in community activities facilitation, Rotation in Medication Administration & Meal preparation, Rotation in Hygiene Support & Life Skills Training, Rotation in program cleaning, Rotation in Therap Data entry, Rotation in checking off on shift tasks as designated in Therap etc. To ensure that the agency is able to bill Medicaid for the services provided, assure that supervisors are running Therap ISP Data report to ensure every shift has inputted data daily (Daily Tracking) on Therap for the service (s) provided to the service recipients directly i.e. personal care goals, ISP/IHP goals. Ensure each program develops and implements a weekly shift assignment schedule outlining daily staff rotations in assignments. Assure recording of data (in Therap) relative to assigned outcomes for service recipients including ISP tracking, Daily Training Record (DTR) etc. Assure completion, review and timely distribution of monthly reports, ISP reporting, financial report etc., for each service recipient, agency petty cash report for each program or any other report as directed. Maintain and account for all financial records and reports of program and service recipients' finances. Ensure effective management of agency petty cash and service recipients' funds, proper documentations appropriate practices consistent with agency policy. Submit and distribute financial reports as directed. Responsible for completing and forwarding the Program's Quarterly Newsletter to Associate Executive Director, Operations for timely review and approval prior to distribution to families, guardians and/or residential providers. Plan and facilitate four meeting opportunities throughout the year (March (Family Fan Hour), June (open house/Show Case), September (Back to Fall Night) and December (End of Year Celebration) for families/guardians to interact with the agency staff in interest of ameliorating the networking and face-to-face interactions between families and agency professionals. Responsible for notifying the Assistant Executive Director, Clinical Services whenever there is a significant change in the individual's status requiring a team meeting. Review periodically the ISP document and report inconsistencies in service recipients' support needs (Ambulation, Self-Care, Behavioral & Medical) to the Associate Executive Director, Operations. Assure thorough dissemination of identified individual needs data into appropriate databases. Assure identified goals are accomplished in accordance with agency and state standards. Scan the Nutrition Audit Report/Findings Report into the E Drive shared folder labeled "Department of Residential & Adult Day Program-Nutritionist Consultant Audit Report" and notifies Residential' s Assistant Directors, Supervisors, Assistant Supervisor, and healthcare supports through groups emails for access, prompt review follow up and/or timely and effective implementation. Assure that the Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral facilitate and implement recommendations made by the Nutrition Consultant promptly. Assure effective coordination and integration of behavioral services of service recipients attending the Behavior Day Habilitation Program. Attend and participate in all meetings, conferences, seminars, and committees as assigned or as necessary for successful achievement of agency and the departmental goals. Assure timely, thorough and effective communications regarding individual served and operations, to all appropriate parties. Assure timely, thorough and effective communications regarding program, employee operations, to all appropriate parties including but not limited to Assistant Director, Day Behavioral Health Supervisor, Day Behavioral Health, Supervisor, Day Behavioral, Support Professional, Day Behavioral Health, Assistant Supervisor, Day Behavioral Health, Support Professional, Day Behavioral and/or Assistant Executive Director, Clinical Services. Responsible for ensuring that, upon notification of qualifying or active incident, appropriate measures are taken consistently with agency policies, procedures and training, and support is directed toward the staff and the service recipient involved in a crisis. Provide guidance, direction, intervention, etc. and ensure that incidents meeting the criteria of Unusual Incident Report are reported through automated UIR/GER documentation, standardized notifications activated as indicated through DDD circular and Arc of Union County trainings. Responsible for ensuring that upon notification of an employee accident at work, appropriate measures is taken consistently with agency policies and procedures. Provide guidance, direction, and assure claim is reported through standardized notification including Employee Accident Report, Root Cause Analysis documentation. Ensure that staff are implementing agency Guidelines for Safe Transporting and Parking for Residential, Transportation, Day Program staff and Agency Employees, Safe Driving Policy. Monitor agency vehicles under direct oversight, assure that they are utilized for the approved agency service or business purpose, within the approved operations timeframes and stored or parked at the approved location. Provide report to the Associate Executive Director Operations on vehicle resource mismanagement or deviations from agency policy. Assure all vehicles are in good status and equipped to receive satisfactory inspection review by Office of Licensing, follow through on vehicle deficiencies featured on inspection/audit reports. Assure that the agency vehicles resource will be utilized for the approved agency service or business purpose, within the approved operations timeframes and stored or parked at the approved location. Review, resolve and follow through on Safe Driving Policy Violations Reports promptly to ensure the safety and welfare of service recipients. These include Escalated Violations Report, Coach Warnings Report, Violations Exceeding Severity, etc. Ensure completion and submission of the Agency Vehicle Accident Report within 24 hours, Police Crash Investigation Report, damage related pictures, and Repair Cost Estimate Report promptly to the HR, and Director of Transportation. Actively support all agency policies and procedures to all internal and external contacts as indicated. Oversee preparation programs for external audits including Day Habilitation Certification review, CCW, CMS, CARF, etc. Have a thorough understanding of Departmental Operational Plan, Agency Strategic Plan, and Human Resource Policies. Assure the appropriate review, timely reporting and investigating of all unusual incidents, allegations, or charges pertaining to individuals served, staff or programs as well as timely and accurate submission of UIR. Seek guidance from the Associate Executive Director, Operations and identify emerging needs of service recipients'/staff/program and develop/coordinate a plan of action to meet those needs within the guidelines of the agency. Assure and facilitate proactive planning for programs relative to unexpected events such as inclement weather, coordinate and communicate decision making to promote service recipients/staff safety and prevent crisis. Lead, develop and update the Emergency Evacuation Plan (EEP) Narrative annually and Evacuation Assessments/Evaluations upon changes on individual's abilities to evacuate/needs promptly with the AD and Supervisor. Assure the EE Plan aligns with the Evacuation Assessment/Evaluation consistently at all times. Develop and distribute an annual Fire Drill Schedule assuring that fire drills are conducted monthly with individuals served present. Develop and distribute an annual Disaster Drill schedule featuring all Types of Disaster Drills including Bomb Threats, Blizzard, Earthquake, Hurricane, Tornado, Utility Failures, Act of Terrorism, Violent Events, and Other Threatening Situations etc. Assure all types of disaster drills are run at least once a year in line with the Disaster Drill schedule assigned by the director including Bomb Threats, Blizzard, Earthquake, Hurricane, Tornado, Utility Failures, Act of Terrorism, Violent Events, and Other Threatening Situations etc. Audit (Medical/Financial, Facility), review, and inspect Behavior Day Habilitation Program to assure operational practices, records and facility systems are consistent with Day Habilitation standards, CARF, DDD, CMS, DHS, CCW standards, and that the program is operating within departmentally established guidelines and is maintaining progressive improvements in designated areas of performance including Quality Assurance etc. Maintain knowledge of DDD, Day Habilitation Certification standards, and CARF regulations and Agency Policies and Procedures. Assure the completion of all required documentation/records and administrative assignments pertaining to service recipients, programs, staff and agency administration in accordance with agency, CARF, DDD, CMS, DHS, CCW and/or other funding sources standards and requirements. Support and assure that programs maintain required certifications, standards and manuals. Facilitate and provide on agency/departmental designed new employee on-boarding communications, expectations and administrative orientations. Facilitate employee Administrative Orientation and ongoing training on agency policies and procedure. Secure the signed orientation documentation, file and distribute as required. Responsible for auditing training records regularly, register staff for training and assuring that all staff members are fully trained in all on program specific trainings, department, agency DDD trainings and meets criteria of a minimum of 12 Hours of Professional Development completed annually, State trainings, etc. Assure proper re-certifications, monitoring attendance and addressing training attendance performance in a timely manner. Seek guidance from the Associate Executive Director, Operations and assure facilitation of employee Administrative Orientation and ongoing training on agency policies and procedure. Ensure the signed orientation documentation is obtained, filed and distributed as required. Complete all supervisory trainings as designated and attend professional development management workshop as directed Complete Routine, Monthly, Quarterly or Business Director's Reports as required and submit promptly as directed. Maintain program certification records, audits and other inspection reports and ensure they are current. Inform Associate Executive Director, Operations and update records in a timely manner regarding guardian's refusal to comply with agency service delivery polices/procedures, provision of required documents, or declination to consent to service delivery, e.g. medical treatment, community integration activities, photograph taking/publication, etc. Seek guidance from the Associate Executive Director, Operations and collect all pertinent referral information, comprehensive vetting of prospective applicants/candidates for a seamless service recipient selection and admission into the Behavior Day Habilitation Program. Seek guidance from the Associate Executive Director, Operations on review of new referrals. Responsible for scheduling and completing an interview with the prospective service recipient and his/her caregiver(s), guardians, etc. and involve the vetting team in the interview and assures that the team determines in advance on the program's ability to serve or not serve the individual and advises the committee during the Vocational Admissions Meeting. Responsible for providing Service Recipient Placement Interview Information to the Associate Executive Director, Operations to update the completed "Placement Referral Summarized Clinical Abstract- Adult Day Services" Form. Actively participate in the Day Habilitation Admissions Committee meeting process; coordinate referral interviews as directed. Responsible for briefing the Associate Executive Director, Operations on all persons recommended for admission. Coordinates with Director (s) for program with vacancy and/or program identified for service recipient's placement customizing the department's standard acceptance letter and sending it to the support coordinator and copying the IDT members and applicable agency cluster support staff as directed. Participate in intake and admission meetings in order to assure appropriate intakes; ensure completion of Behavior Day Habilitation Program pre-admission documentation and secure all new admission documents consistent with licensing, state and agency policies. Assure all required information for service recipient's placement and service delivery is obtained. Participate in Ad hoc agency committee (s) as assigned. Facilitate training as assigned. Initiate "on call" procedures with respect to arising incidents including 911 emergency calls, life threatening incidents, and/or operational breakdown etc. and complete Unusual Incident Reports (UIR) in a timely manner consistent with agency policy and state law. Responsible to report to immediate Supervisor or through the chain of command any suspected incidents of abuse, neglect or exploitation of service recipients and complete Unusual Incident Reports (UIR) in a timely manner consistent with agency policy and state law. Monitor and safeguard agency property and funds. Perform duties according to the methods, manner and spirit of the ideals of the field of Human Services and within the profession/job specific guidelines, including wearing appropriate attire etc. Must fully cooperate with all investigations, inspections, etc. internally and externally including full cooperation with the Department of Human Services, the Division of Developmental Disabilities, or any other duly authorized person regarding the care and service given to individuals' served by this Agency.
