Physician - Granada Hills, CA Vivo HealthStaff is recruiting for a High Risk Clinician in Granada Hills, California. This is a permanent position with salary and benefits. About the Opportunity Our client, a rapidly growing healthcare provider in Southern California, is seeking a High Risk Clinician to provide comprehensive clinical care to high-risk patient populations in the San Fernando Valley and surrounding areas. This role leads post-discharge transitional care efforts, ensuring clinical stabilization, reducing preventable hospital utilization, and improving patient outcomes. Direct patient care is provided in a clinic setting with potential for home and community visits as needed. Key Responsibilities Provide clinical care to complex, high-risk patients in a designated clinic, with periodic community visits when necessary. Lead post-discharge transitional care efforts, working closely with Primary Care Physicians and specialists. Participate in and lead interdisciplinary meetings to support patient care and ensure effective communication among healthcare providers. Oversee the clinical work of a High-Risk Nurse Practitioner conducting in-home visits. Refer patients to appropriate clinical, social, and behavioral health resources. Document all care provided in compliance with regulatory standards. Adhere to clinical guidelines, group protocols, and health plan requirements. Perform additional duties as assigned by management. Qualifications Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO). Board Certified/Board Eligible in Internal Medicine, Geriatrics, or Emergency Medicine (preferred but not required). Active California medical license and DEA privilege. Minimum of five years of clinical experience, with two years of managed care or health plan experience preferred. Proven excellence in both hospital-based and outpatient medical practices. Strong clinical knowledge, communication, and organizational skills. Ability to work in a fast-paced environment, independently and collaboratively. High personal ethics and the ability to build positive relationships with management, medical directors, and department heads. Compensation and Benefits Salary: $270,000 - $300,000 annually, based on experience, skills, and location. Additional Compensation: Potential sign-on bonus and discretionary awards.
