Position Summary ORTHO/NEURO NURSE NAVIGATORWiregrass Ranch Hospital serving OHJOIOHJOI PATIENT NAVIGATORSFull TimePosition Summary OHJOI Navigators support 2-3 physicians each and follow their patients from the start of the surgery schedule, through discharge, and up to the 21-day check-in. Wiregrass Ranch Hospital will be opening in the Spring of 2026 with anticipated volumes to be over 2300 annually for inpatient and outpatient ortho and spine surgical cases. With these volumes, there is a need for 3 Nurse Navigators to provide the necessary level of support for these patients as this hospital opens, and our program begins in this market. One Nurse Navigator will Support Knee volumes at approximately 678 cases annually, One Nurse Navigator will support Hip Volumes at approximately 648 cases annually, and these two will split the shoulder volumes which are around 133 cases annually. The spine volumes are around 630-840 cases approximately which will need the support of 1 Navigator to begin. Education/Training An Associate of Science in Nursing (ASN) is required. A Bachelor of Science in Nursing (BSN) is preferred. Licensure/Certification • Maintains license as an RN in the state of Florida. • Maintains current BLS/healthcare provider certification. Experience Minimum of two (2) years of clinical experience in acute care is required. Ortho/Neuro experience is preferred. Prefer candidates in Wesley Chapel for Wiregrass Ranch Hospital LocationEssential Functions • Coordinates patient care through the continuum. Assists with timely access to care, coordinates/facilitates appointments and services, and encourages adherence to treatment plans, protocols, and follow up care. • Assesses patients' current and future needs, provides access to resources (internal and external), and makes appropriate referrals as needed. • Provides education and information to patients and families related to the healthcare system, multidisciplinary team member roles, plan of care, and available resources. • Establishes and maintains close collaboration with physicians and multidisciplinary team members to ensure seamless patient care and care coordination. • Tracks determined program metrics and quality indicators. Documents patient interactions and progression, and communicates navigation activities to physicians, the care team, and leadership. • Collaborates with the clinical team to obtain or develop patient and family educational materials and resources. • Demonstrates knowledge, skill, and coordination to provide nursing care and guidance to patients from screening to post-operative care and follow up. • Demonstrates effective communication with patients, families, peers, members of the multidisciplinary healthcare team, and representatives of community organizations and resources. • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards. • Maintains compliance with all Orlando Health policies and procedures
