Summary: Urology call coverage for Level II Trauma center and Hospital BID DUE DATE: 02/12/25 5pm EST Rate Caps for Submission: Malpractice - Must be included in rates. Orientation -- half the hourly/callback ratekicks in after 8 hours. "Per hour of orientation or training beyond 8 hours" Holiday - Must be 1.5x the regular rate"Per holiday hour, no OT" On Call (Daily) -and below - "Per 24 hours to include 4 hours gratis" 24 Hour Holiday Call Rate -- "Per 24 hours to include 4 hours gratis" Call Back -and below - Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms." Holiday Call Back -- "Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms." Schedule/Availability Requirements: April 25 @ 5p through April 28 @ 7a May 23 @ 5p through May 27 @ 7a June 06 @ 5p through June 09 @ 7a Facility Location: Good Samaritan Setting: Emergency Dept./Surgery FTE: 1 Duties: Cover Urology call - get handoff from Urologists if appropriate Required Procedures: General Urology EMR: EPIC Support Staff: Surgery nurses and techs. on call Reason for Coverage: previous locum scheduled cancelled stint Deal breakers (do not present this): Not willing to learn EPIC Travel Preferences: Preferred Lodging: Candlewood Suites Air Travel: Local, preferred but not required Credentialing Information: Timeframe: 3-4 weeks minimum Requirements: M.D. or D.O.; DEA; NE license; case logs from last 2 years Credentialing with other facilities? No Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less?: No - they will need to be enrolled with our payors so that we can bill under their own name. Additional Details / Questions Answered by Facility: On average, how busy is the call volume per day? (Consults vs. callbacks?): Will have days with no consults for urology out of the ED. However, there are other days where there may be 4 or 5 Is the Urologist responsible for admissions or will the hospitalists be covering admits? Hospitalists admit patients CSH history? Please state in provider submission page if so SUBMISSION REQUIREMENTS Board Certified/Board Eligible (Urology) - REQUIRED, NOTE AT NAME CLEAR. Active NE license - HIGHLY PREFERRED Clean Malpractice - HIGHLY PREFERRED Availability of dates in the job description - REQUIRED, NOTE AT NAME CLEAR. Case logs are required - REQUIRED, INCLUDE AT PRESENTATION. Disclose if the provider has worked at any other CHI/Dignity facility - REQUIRED, NOTE AT NAME CLEAR.
