What You'll Do Perform comprehensive assessments in the patients' homes and identifies patient's needs Develop a care plan in collaboration with patient/CGs, healthcare provider, case manager, and/or supervisor as assigned and document appropriately Assess the need for additional services and equipment on admission on an ongoing basis and inform the case manager and/or supervisor on a timely basis Assess the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effects Take verbal orders from the healthcare provider as necessary promptly and documents appropriately Initiate appropriate preventative, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients/CGs and staff promptly Demonstrate professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues Updates to patients' clinical records including medication profiles, plan of care, OASIS, case conference, progress notes, and re-certification documentation as required Serve as a preceptor when needed Who You Are Compassionate, dependable, and patient-focused Comfortable working independently in the home setting A collaborative team player who values communication and support Motivated by meaningful, one-on-one patient care Qualifications Graduate of an accredited nursing program (RN) Active state RN license and current CPR certification Minimum 1 year of clinical nursing experience (home health preferred, not required) Valid driver's license, reliable transportation, and auto insurance Strong interpersonal and communication skills Ability to perform skilled nursing assessments and care in the home setting Experience with Home Care Home Base (HCHB) documentation preferred (training available) Pay & Perks Competitive pay that reflects your experience and expertise Full-time benefits (23 days of PTO, medical, dental, vision, and disability coverage) available to full-time associates Flexible scheduling designed to support true work-life balance (available to full-time, part-time, and per diem clinicians) Mileage reimbursement for patient-related travel Continuing education opportunities Generous referral bonus program Oasis Virtual Assistant to streamline documentation Supportive, tenured leadership team that truly has your back Our Mission & Values Our mission is to deliver exceptional home health care through clinical excellence, compassion, and integrity, while making a meaningful impact in the communities we serve.
