Welcome to Cherry Health - video about Cherry Health RN Behavioral Health Care Manager is responsible for comprehensively developing, implementing, monitoring, and evaluating patient-centered care plans for a targeted patient population. As part of an expanded health care role, the Behavioral Health Care Manager facilitates collaborative care and coordination across healthcare providers and systems, as well as community and social supports to ensure the delivery of quality, efficient, and cost-effective health care services. This position utilizes therapeutic communication and psychoeducation to support patients and their psychiatry care team to achieve treatment and recovery goals. This is a full time, benefit eligible position typically working Monday - Friday 8a-5p onsite at our Heart of the City Health Center. Responsibilities: Identify targeted high-risk population per provider referral, risk stratification, and patient lists. This includes patients with repeated social and /or behavioral health crises. Assess over time the behavioral health care, educational, and psychosocial needs of the patient/family utilizing standardized assessment tools such as depression screening, functionality, and suicide risk assessments. Collaborate with primary care providers, patients, and members of the health care team, including continuum of care settings and community to develop a comprehensive individualized plan of care and targeted interventions. Continually monitor patient/family response to plan of care and revise the care plan as indicated. Provide psychoeducation and motivational interviewing with a focus on empowering the patient/family to build capacity for self-care. Implement systems of care that facilitate close monitoring of high-risk patients to prevent and /or intervene early during acute exacerbations. Facilitate interventions, safety planning, and/or referrals for patients who are in crisis situations. Serve as liaison to psychiatric hospitals, crisis stabilization, community mental health, and specialty pharmacies. Provide follow-up with patient/family when patient transitions from one setting to another. Complete and maintain required documentation of timely post-hospital follow-up, medication reconciliation and patient education, care coordination, and other nursing interventions as indicated. Collaborate with Cherry Health's leadership to continuously evaluate process, identify problems and propose/develop process improvement strategies to enhance behavioral health care management and psychiatric services. Understand and adhere to Cherry Health's compliance standards as they appear in Cherry Health's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy. Keep abreast of all pertinent federal, state and Cherry Health regulations, laws, and policies as they presently exist and as they change or are modified. Other duties as assigned. Qualifications: Education: Degree as RN either Associates or Bachelors. Licensure with the State Of Michigan as a Registered Nurse required. Completion of self-management support training preferred. Experience: Three years of nursing experience with mental health and/or substance abuse disorders preferred or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above. Skills / Knowledge / Abilities: Basic Life Support/CPR certification as required by license and certification. Strong communication and interpersonal skills. Ability to work independently while maintaining self-direction. Working knowledge of computers and their applications preferred. Ability to work efficiently under tight deadlines and high-volume conditions. Frequent telephone and computer use, requiring manual dexterity for writing and computer operation. Ability to sit, stand, or walk for extended periods. Vision requirements may include close vision, distance vision, and the ability to adjust focus. May require bending, squatting, kneeling, and lifting up to 50 pounds. Use of Personal Protective Equipment (PPE) as required.
