The Social Worker 2 role within the University Hospitals Cleveland Medical Center offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. This position adds value to patient care and significantly assists patients with various issues. We currently offer an opportunity for a PRN, Social Worker to support pediatric and OB patients in Cleveland. A Brief Overview Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves a resource to others in work-team and department. What You Will Do Provides psychosocial assessment / intervention to assist patients/families (including clinically complex) • Provides age specific care to patients/families in all age groups. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs. • Develops plans with attention to individual patient and family goals • Provides linkage to community resources and UH network services to facilitate optimal care Communicates/collaborates with the work team using a multidisciplinary approach. Provides guidance/ consultation to peers/team regarding routine psychosocial issues and /or discharge planning issues. Provides supervision to social work interns if LISW eligible (completion of supervision hours). Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Qualifications: Education Master's Degree from an accredited school of Social Work (Required) Work Experience Hospital/Health Agency experience. (Preferred) Licenses and Certifications Licensed Social Worker (LSW) (Required) Licensed Independent Social Worker (LISW) within 1 Year (Preferred) Preferred Qualifications/Position Details At least 1 year of prior work with pediatrics and/or OB is strongly preferred. This social worker is needed to cover Rainbow Babies & Children's units, including the NICU, and OB at our main campus location. They will also ideally float to other locations, such as outpatient clinics and any of our community hospitals that offer these services. The role of the social worker is critical to provide psychosocial assessments, crisis interventions, and resource referrals, as well as timely, efficient, safe discharge planning, and supportive counseling services to meet the needs of patients and families. Examples of issues may include domestic violence, child abuse, poverty, depression, homelessness, behavior problems, teen pregnancy, etc. The PRN social worker will cover PTOs and FMLAs. Shifts are 8:30am-5:00pm; weekdays are the greatest need. TRAINING : you must be available 2-3 days per week to train over the course of a month.
