Description Join our dedicated team as a Per Diem Day Care Manager RN, where you will play a vital role in providing exceptional, patient-centered care management for at-risk patients in an acute care setting. Your responsibilities will include comprehensive assessment and planning, coordination of care, utilization management and review, discharge planning, documentation of interventions, ensuring regulatory compliance, and acting as a patient advocate. Required Qualifications: Associate's Degree in Nursing upon hire California Registered Nurse License upon hire A minimum of 2 years of experience in an Acute care environment, preferably in a Medical Surgical or Inpatient setting At least 1 year of experience in care management or utilization review in any setting, or completion of TIPS program or Case Management Orientation Program upon hire Preferred Qualifications: Bachelor's Degree in Nursing or higher within 3 years of hire Case management certification (ACM or CCM) Why Join Providence?
