Home Care Nurse Practitioner - San Francisco Description We are seeking a highly skilled and compassionate Home Care Nurse Practitioner (NP) to join our team at Health Link Home Health & Hospice. Our Values Put Patients First Empower Clinicians with Support and Independence Operate with Compassion and Integrity Foster Collaborative Teamwork Provide Stability and Excellence What You'll Do Patient Care & Assessment: Conduct comprehensive health assessments, physical examinations, and diagnostic evaluations for patients in their homes. Develop individualized care plans tailored to the patient's needs, considering their medical history and functional status. Prescribe and manage medications and treatments as permitted by licensure and scope of practice. Utilize community resources to design and direct care that meets the holistic needs of patients and their families. Palliative Care Management: Provide comprehensive palliative care to improve comfort, alleviate pain, and address psychological and emotional needs. Use a patient-centered approach to discuss end-of-life issues and disease progression with patients and their families. Monitor and adjust palliative interventions based on patient needs, ensuring a holistic approach to care. Collaboration & Coordination: Oversee the day-to-day operations of the Palliative Care Program, ensuring quality care is delivered to all patients. Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to provide seamless care delivery. Make referrals or consult with other specialists to ensure holistic, multidisciplinary care is provided. Assist in the orientation of new personnel and promote the ongoing clinical development of our nursing staff. Clinical Documentation & Reporting: Accurately document all patient interactions, assessments, and progress in the electronic health record system. Maintain confidentiality and comply with all relevant healthcare regulations and standards. Communicate regularly with the San Francisco back-office team to coordinate logistics and ensure continuous patient support. Qualifications Active California RN License and current Certified Nurse Practitioner (NP) status. Excellent skills in assessment, diagnosis, and treatment. Strong interpersonal communication skills, with a compassionate and empathetic approach to patient care. Highly organized, self-directed, and motivated with the ability to work independently in a home setting. Ability and willingness to travel to patient homes throughout the Greater San Francisco Bay Area. You'll be great for this role if you have: Education and/or strong clinical experience in palliative care, hospice, oncology, or geriatrics. Experience in coordinating care teams and working with interdisciplinary healthcare providers. Proficiency in managing chronic conditions and providing end-of-life care. Environmental Job Requirements, Working Conditions, and Benefits This role involves traveling to patient homes in the Greater San Francisco Bay Area, supported by our local administrative office. The compensation for this role is $160,000 to $200,000 annually, plus a $10,000 Sign-On Bonus.
