In-Home Care Registered Nurse (RN) Needed

Company Description Right at Home is a private duty, non-medical in-home care agency that provides comprehensive care planning, supervision, and care management to those within our community. Role Description This is a part-time or PRN hybrid role for a Nurse Care Coordinator, located in Westborough, MA, with some work-from-home hours. The Nurse Care Coordinator will manage day-to-day care coordination as needed, new client intake meetings and creating detailed plans of care. Responsibilities include developing and updating care plans, communicating with healthcare professionals, clients, and family members, coordinating home care services, and providing exceptional customer service to ensure quality care is delivered. The role requires a focus on promoting health and well-being among older adults in various care settings.
