Job Description The APP Team Lead in Bone Marrow Transplant plays a key role in delivering comprehensive, family-centered care to children undergoing hematopoietic stem cell transplantation. This advanced practice provider works across both inpatient and outpatient settings, managing the full continuum of transplant care. The APP Team Lead provides direct clinical management, patient and family education, coordination of multidisciplinary services, and performs specialized procedures essential to transplant care. The APP Team Lead is accountable for direct oversight of all APPs (Physician Assistants and Advanced Practice Registered Nurses) within their clinical department. The APP Team Lead will provide direct APP clinical supervision, develop and implement the APP work schedule, develop and monitor APP annual performance goals and objectives, provide APP education and professional development, and encourage advancement within the APP Career Ladder. The APP Team Lead will assist with new APP recruitment, hiring, and coordinate on-boarding activities to ensure successful transition to practice. ResponsibilitiesClinical Care - Inpatient & Outpatient Manage daily care of pediatric BMT patients in both inpatient units and outpatient clinics. Conduct comprehensive assessments, develop treatment plans, and adjust therapies in collaboration with the transplant team. Monitor for and manage transplant-related complications, including graft-versus-host disease, infections, organ dysfunction, and treatment toxicities. Provide continuity of care throughout the transplant process, from pre-transplant evaluation through long-term follow-up. Coordinate care transitions between inpatient and outpatient settings to ensure seamless patient experiences. Patient & Family Education Deliver ongoing education to patients and families regarding transplant processes, medications, symptom management, infection prevention, and long-term expectations. Support families in understanding complex medical information and making informed decisions. Develop and update educational materials and participate in teaching sessions, classes, or support groups. Procedures Perform advanced procedures such as lumbar punctures, bone marrow aspirates/biopsies, bone marrow harvest, and other skills within scope of practice. Assist with or independently perform diagnostic and therapeutic interventions as needed. Ensure procedural safety, informed consent, and appropriate follow-up care. Collaboration & Coordination Work closely with physicians, nurses, pharmacists, social workers, and other specialists to deliver integrated transplant care. Participate in multidisciplinary rounds, care conferences, and quality improvement initiatives. Communicate effectively with referring providers and community partners. Leadership Responsibilities Develop, implement, coordinate departmental APP clinical work schedules including clinic/office/operating room, on-call, sick call, and holiday and PTO. Ensure PTO requests meet department needs. Oversee and problem solve APP daily work related issues. Develop and monitor APP annual performance goals and objectives, provide and support APP education and professional development. Participate in teaching, learning, and peer review. Be knowledgeable about the APP Career Ladder document, the competencies within each ladder level, and the promotion request process. Encourage advancement of APPs within the APP Career Ladder. Assist with new APP recruitment, hiring, and coordinate on-boarding in their clinical department to assure successful transition to practice. Quality, Safety & Professional Development Adhere to institutional protocols, regulatory standards, and evidence-based transplant guidelines. Participate in research, data collection, and quality improvement projects related to BMT outcomes. Maintain advanced practice certification and engage in ongoing professional education in pediatric oncology and transplant medicine. Performs other job-related duties as assigned QualificationsEducation and/or Experience Required: Education: Advanced Practice Nurse Practitioner: Graduate degree in nursing accredited by one of the national certifying bodies in nursing. (Or, on or before December 31, 2004, completion of an advanced nurse practitioner program recognized by a national certifying body for certification as a nurse practitioner. Physician Assistant: Graduation from a physician assistant program accredited, at the time of graduation, by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA). Experience: At least 5 years APP experience Education and/or Experience Preferred: Experience: 7- 10 years APP experience and 1-2 years supervisory experience. License and/or Certification Required: Advanced Practice Nurse Practitioner: Holds a current Connecticut registered nurse license from Connecticut Department of Public Health. Hold and maintain current certification as a nurse practitioner, clinical nurse specialist, or nurse anesthetist from one of the following national organizations: Physician Assistant: Certified by the National Commission on Certification of Physician Assistants, Inc. (NCCPA). Licensed by the Connecticut Department of Public Health. Knowledge, Skills and Abilities: Knowledge: Institutional computer software including MS Office, EPIC, SABA/Halogen Performance Management. Medical content. knowledge of adequate level to perform the clinical role competently. Knowledge at the APP Career Ladder IV competency (Expert & Coach). Skills: Communication, customer service, providing pediatric healthcare in a variety of settings (urban, suburban, rural). Skills at the APP Career Ladder IV competency (Expert & Coach). Abilities: Work independently. Manage complex family issues and provide patient/family teaching and discharge planning. Work flexible hours/days to meet patient needs, programs, and services.
