This may include assessing, evaluating, and updating therapeutic/rehabilitative/functional status, and participating in the development of the total plan of care. Responsibilities: Assessing and evaluating the home environment for muscle strength, mobility, gait, ROM, and potential for rehab. Instructing patients/clients and families/caregivers in the use and care of therapeutic appliances and determining priority needs for physical therapy. Reporting the patient's/client's reaction to treatment or changes in condition to the physician. Initiating physical therapy programs and instructing other personnel and/or family/caregiver members in certain phases of physical therapy. Supervising homemaker/Home Health Aides when participating in the patient's/client's rehabilitation regimen. Preparing and submitting clinical and progress summaries based on goal attainment and reporting information on the patient's condition to the attending physician. Providing physical therapy consultation to home families/caregivers when indicated and participating in in-service education programs for nursing agency personnel. Complying with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other regulations and licensure requirements relevant to the position. Requirements: 2 years experience in a healthcare setting required, home health experience including OASIS preferred. Required certifications: BLS & NPI #; CA PT license. Experience with EMR HomeCare Home Base (HCHB). Current driver's license & proof of automobile insurance. Strong oral and written communication skills and good organizational skills. Need 2 supervisory references at submission, 1 within 12 months & 1 within 3 years of work experience. Candidates may not have been directly employed by any facility associated with CommonSpirit within the past year.
