JOB SUMMARY The Outreach and Engagement Specialist plays a critical role in the success of the Roseland HTC program by building relationships and increasing community access to healthcare and support services. The Engagement Specialist will conduct proactive outreach to individuals in the community, including those with recent emergency department visits or post-overdose encounters, with the goal of facilitating care re-engagement and service connection. JOB DUTIES Provide community-based outreach and engagement through canvassing and promotion of services. Connect with individuals who are high utilizers of emergency services or recovering from overdoses to initiate support. Complete outreach triage assessments and follow-ups to ensure appropriate referrals are made. Maintain strong documentation and submit daily service hour reports with supporting materials. Conduct warm hand-offs to care providers and support transportation to intake appointments when needed. Develop and maintain relationships with community members and mutual referral sources. Participate in regular staff meetings, training, and professional development. Assist clients in navigating and re-engaging with services as needed. Perform other duties as assigned by supervisor. QUALIFICATIONS High School Diploma or GED required. At least one year of related experience in community outreach, social services, or peer support. Strong communication and interpersonal skills. Familiarity with community resources and a demonstrated ability to engage diverse populations. Must have a valid driver's license and vehicle insurance. Proficiency in basic computer use and documentation tools. Must be eligible for CRSS (Certified Recovery Support Specialist) certification within one year of employment. WORKING CONDITIONS AND PHYSICAL REQUIREMENTS Regular community-based work with moderate physical activity. Frequent walking and standing; occasional lifting (up to 25-50 lbs). May occasionally work evenings/weekends based on community needs. COMPENSATION & BENEFITS Hourly Rate: $18.02 Comprehensive Benefits Package, including: Medical, Dental, and Vision Coverage Paid Time Off (PTO) & Holidays Professional Development Opportunities Employee Assistance Program (EAP) Occupational Risk for Exposure to TB: There is an occupational risk for exposure to TB, as this position involves extensive community and patient contact. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections.
