Title: T-LPN Location: 50 Pheasant Rd, Peterborough, NH 03458 Shift: 7am-3:30 pmContract:9 weeksGuaranteed hour: 40Job Summary: We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our long-term care facility team. The LPN will provide high-quality nursing care to residents in accordance with facility policies, state regulations, and the individual care plans of each resident. The ideal candidate will have a strong understanding of geriatric care and a commitment to enhancing the quality of life for elderly residents. Key Responsibilities: Administer prescribed medications and treatments in accordance with approved nursing techniques and facility policies. Monitor and document residents' health status and report any significant changes to the RN or physician. Assist with the development and implementation of individualized care plans. Provide direct patient care, including wound care, catheter care, and assistance with activities of daily living (ADLs). Maintain accurate, complete, and timely nursing documentation. Supervise and support Certified Nursing Assistants (CNAs) and other support staff. Ensure compliance with infection control policies and safety standards. Communicate effectively with residents, families, and interdisciplinary team members. Participate in shift reports, training sessions, and quality improvement initiatives. Qualifications: Current and valid LPN license in the state of employment. CPR/BLS certification (required). At least 2 year of nursing experience in a long-term care or similar healthcare setting Strong organizational, interpersonal, and communication skills. Ability to work flexible hours, including weekends and holidays. Familiarity with electronic medical records (EMR) systems (preferred).
