This position delegates responsibility to the Physical Therapist Assistant and aides. Activities of this position are patient-focused and utilize assessment, planning, intervention, and evaluation in accordance with physical therapy standards and physician orders. Evaluates the patient's function by performing formal and informal testing measures. Assesses skeletal, neurological, respiratory, and cardiovascular status. Develops a goal-specific treatment regime based on assessment data and the patient's tolerance. Submits a written treatment regime as part of the plan of treatment authorized by and coordinated with the physician. Updates the plan of care as needed to reflect changes in patient status. Co-signs verbal orders received by the Physical Therapist Assistant as necessary. Devises a schedule of treatment, which is frequency and duration specific, to be discussed with the patient, family, and caregiver. Educates the patient, family, and/or caregiver regarding pathology or condition, patient alternatives, prognosis, and rationale for treatment through forms of rest, exercise, modalities, and other indicated interventions. Provides supervision for Physical Therapist Assistants, Physical Therapy Aides, Home Health Aides, and students. Recommends adaptive or supportive equipment based on assessment. Contacts appropriate representatives or suppliers to place orders and assists in securing equipment for the patient. Documents the patient, family, and caregiver response to treatment and changes in patient condition per documentation policies. Completes daily documentation necessary per policy and in compliance with regulatory agencies. Provides treatment intervention and demonstrates competence to facilitate or restore optimal functioning. Educates patients, families, and caregivers in home education programs using relevant and effective teaching methods. Demonstrates effective oral and written communication with all internal and external customers to maximize quality of patient care. Promotes education and development among staff, students, and other healthcare providers. Assists with program development and participates in team conferences and interdisciplinary meetings. Promotes physical therapy and participates in community education. Functions as case manager for assigned cases. Demonstrates commitment to the mission and values as evidenced by customer service and teamwork. Maintains confidentiality of patient information in accordance with policies and procedures. Other duties as assigned. Demonstrates competency in Microsoft Windows and is required to participate in further learning opportunities. Interpersonal skills are needed for effective verbal and written communication in English. Ability to work independently, demonstrating problem-solving skills, organizational skills, time management skills, and initiative. Ability to work in environments where exposure to communicable diseases may occur. Ability to use personal protective equipment correctly. Must be able to move freely about the facility and perform the job functions as outlined. Overall vision and hearing are necessary with or without assistive devices. Regularly required to sit, stand, walk for long periods, and may require frequent postural changes. Some exposure to bloodborne pathogens and other potentially infectious materials occurs.
