About the Role We are seeking a dedicated and compassionate Physical Therapist (PT) to join a leading Skilled Nursing Facility (SNF) in Petaluma, CA. The ideal candidate will provide high-quality rehabilitation services to help residents regain strength, improve mobility, and achieve optimal physical function. Key Responsibilities Conduct comprehensive patient evaluations to assess physical abilities and create individualized treatment plans. Deliver physical therapy interventions to enhance mobility, strength, and daily functioning. Educate patients and families on exercise programs, mobility strategies, and safety techniques. Collaborate with an interdisciplinary care team-including physicians, nurses, and occupational therapists-to ensure coordinated patient care. Track and document patient progress, updating treatment plans as needed. Maintain compliance with regulatory standards and facility policies. Qualifications Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution. Active Physical Therapy license in California (must be current prior to start). Minimum 1 year of experience in a skilled nursing facility or similar setting preferred. Strong knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment. Excellent communication, documentation, and interpersonal skills. Empathetic and patient-centered approach to rehabilitation.
