Position Details & Responsibilities Manage an average caseload of 7-9 patients per day in a collaborative SNF setting. Patient Care - Conduct evaluations, establish treatment plans, and manage a diverse caseload, utilizing strong Part B skills. Advanced Practice - Proficiency in contracture management and creative use of modalities (e.g., electrical stimulation, heat tissue warming, static progressive positioning). Program Growth - Interest in obtaining Wound Care Certification through ACP and utilizing adjunct wound healing therapies (diathermy, E-STEM). Teamwork - Supervise assistants/aides, collaborate with the interdisciplinary team, and participate in group treatment interventions. Documentation - Complete timely and accurate documentation using Net Health/Optima and Point Click Care (PCC).Export to Sheets Requirements: Education: Graduate of an approved PT curriculum (DPT preferred, not required). Licensure: Active, unrestricted CA license (willingness to obtain). Skills: Strong Part B skills and interest in wound care/modalities. Qualities: Flexible, team player, professional, and open to feedback and learning. CPR certification. We offer free CEUs and resources through our ACP University contract.
