Facility: BayCare HomeCare Location: Lakeland, FL Status: Full Time Shift Hours: 8:00 AM - 5:00 PM / Monday - Friday / 1 Weekend Rotation a month. Home visits in assigned service area: Polk County Weekend Work: Occasional On Call: Yes BayCare offers a competitive total reward package including: Benefits (Health, Dental, Vision) Paid time off Tuition reimbursement Mileage reimbursement 401k match and additional yearly contribution Yearly performance appraisals and team award bonus Community discounts and more The Home Health PTA provides hands-on care in the home for a group of patients consistently in accordance with the principles and practices of physical therapy.Responsibilities include Assists Physical Therapist with the administration of interventions utilized in physical therapy and rehabilitatio May progress patients in the treatment plan developed by the physical therapist Communicates with all physicians involved in the plan of care and other healthcare practitioners (as appropriate) related to the current plan of care Follows established procedures, emergency and safety precautions at all times in the treatment of patients and use of modalities Maintains timely communications, records, reports, and other documentation in accordance with agency policies & procedures, and Medicare guidelines Captures documentation during the home visit following company policy using company issued laptop Assists in progressing therapy programs with prior assessments and approval, prepares clinical progress notes and participates case conference and utilization reviews Required Certifications and Licensures: Physical Therapist Assistant State of Florida Licensure Driver's License State of Florida (Florida driver's license is required within 30 days of hire) Basic Life Support Education: Associates Degree Physical Therapy Assistant Experience: 1 year Physical Therapy Assistant experience Preferred 1 year Home Care Why join BayCare HomeCare?
