About the POSITION: A well-established community health center in Western New York is seeking a dedicated Family Nurse Practitioner to join its team at a primary care site located in the heart of Medina. This full-time position offers a balanced schedule and the chance to work alongside a small, collaborative team of providers. Schedule: 40 hour work week. Flexible schedule. Clinic is open: Monday, Tuesday, Wednesday, Friday: 8:00 AM – 5:00 PM Thursday: 8:00 AM – 8:00 PM Responsibilities: Provide full-spectrum primary care services to a diverse patient population Collaborate closely with one other experienced FNP and an MD/DO in a team-based care model Conduct health histories and physical examinations Diagnose conditions and develop comprehensive treatment plans Prescribe medications in accordance with NYS guidelines Perform minor office procedures Provide counseling and patient education Refer patients to appropriate specialty care as needed About the COMMUNITY: Nestled between Buffalo and Rochester, Medina, NY is a charming village with a historic downtown, scenic canal views, and a strong sense of community.
