Job Description We are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide ongoing behavioral health services to residents in skilled nursing and rehabilitation facilities throughout Tompkins County, NY. This role focuses on continuity of care, routine follow-ups, and collaborative treatment planning in a non-hospital, supportive environment. RequirementsConduct comprehensive psychiatric evaluations and follow-up assessments. Develop individualized treatment plans focused on stabilization and symptom management. Prescribe, adjust, and monitor psychiatric medications. Communicate treatment recommendations with facility leadership, nursing teams, and families. Maintain accurate and compliant documentation in EMR systems. Promote resident-centered, trauma-informed psychiatric care. Active New York State license as a Psychiatric Mental Health Nurse Practitioner (PMHNP). Board Certified PMHNP. Strong clinical judgment, communication skills, and ability to collaborate with interdisciplinary teams. Prior experience working in nursing homes, geriatrics, rehabilitation, or long-term care is preferred but not required. Experience as a Psychiatric RN or NP is a plus.
