Description The Director of Continuous Quality Improvement (CQI) is responsible for leading and advancing a comprehensive quality improvement and quality assurance system across Valley View Health Centers. This role partners with clinical and operational leadership to improve patient outcomes through data-driven decision-making, root cause analysis, continuous improvement, and change management. The Director ensures compliance with regulatory standards, supports site visit readiness, and fosters a culture of quality, accountability, and excellence across all health center operations. Essential Functions Functional Area: Continuous Quality ImprovementEstimated Effort: 50% Responsibilities, Deliverables, Outcomes and Expectations: Serve as co-chair and primary facilitator of the Continuous Quality Improvement (CQI) team Lead CQI initiatives using root cause analysis, collaborative problem solving, and change management Facilitate CQI-related improvements across clinical and operational processes Identify quality assurance and quality improvement KPIs and data sources; monitor trends and outcomes Prepare and present quarterly CQI reports to the CAC Governing Board Promote and sustain a culture of Continuous Quality Improvement across all health center locations Monitor implementation of process improvements to ensure consistent, high-quality patient experiences Ensure implementation and ongoing evaluation of QI/QA policies, procedures, and operating standards Ensure quarterly QI/QA assessments are completed in accordance with HRSA requirements Collaborate with clinical and operational directors to improve processes impacting patient health outcomes Maintain clear documentation of CQI initiatives, outcomes, and achievements Establish a systematic approach to CQI including assessment, planning, training, implementation, coaching, and sustainment Develop and oversee CQI work plans to achieve quality goals and improve efficiency and effectiveness Monitor results against benchmarks and identify opportunities for improvement when goals are not met Functional Area: Compliance and Site Visit ReadinessEstimated Effort: 20% Responsibilities, Deliverables, Outcomes and Expectations: Monitor HRSA CQI and program monitoring requirements to ensure ongoing compliance Maintain responsibility and accountability for HRSA Compliance Manual Chapter 10 (Quality Improvement/Assurance) Maintain responsibility and accountability for HRSA Compliance Manual Chapter 18 (Program Monitoring and Data Reporting Systems) Assess policies, procedures, and practices to ensure continuous site visit readiness Collaborate with staff to resolve compliance-related issues Participate in on-site visits and audits as needed Work collaboratively with compliance teams to ensure HIPAA protections and safeguarding of PHI Identify issues related to data collection methods and ensure compliance with accreditation and regulatory standards Functional Area: People ManagementEstimated Effort: 20% Responsibilities, Deliverables, Outcomes and Expectations: Provide direct supervision and guidance to the EHR Specialist Monitor, evaluate, and guide performance of direct reports Collaboratively establish goals and monitor progress Provide coaching, feedback, and performance development support Delegate tasks appropriately to ensure efficiency and accountability Functional Area: Agency Service and Other DutiesEstimated Effort: 10% Responsibilities, Deliverables, Outcomes and Expectations: Serve on CAC and Valley View Health Centers committees and teams as assigned Participate in special projects as directed Minimum Requirements Education: Bachelor's degree in social science, public health, or a closely related field. Job-Related Experience: Minimum five (5) years of experience in data collection, analysis, reporting, and quality improvement methodologies Minimum five (5) years of experience leading teams and facilitating multidisciplinary groups People Management Experience: Minimum three (3) years of direct supervisory experience Licenses, Certifications, and Accreditations: None required. Preferred Qualifications Experience leading Continuous Quality Improvement initiatives in a healthcare setting Prior experience working in a Federally Qualified Health Center (FQHC) Working Conditions This position operates primarily in a climate-controlled office environment, with additional work performed in clinical settings. Duties require regular sitting, computer use, and communication. Occasional standing, walking, and lifting up to 25 pounds may be required. Work is fast-paced and involves managing multiple priorities and complex quality initiatives.
