Licensed Clinical Social Worker LCSW

Job Description We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our healthcare team. In this role, you will provide counseling, support, and case management services to patients and their families. The ideal candidate will have strong clinical skills, excellent communication, and a passion for helping individuals navigate emotional, behavioral, and social challenges. Responsibilities: • Conduct psychosocial assessments and develop individualized care plans • Provide individual, group, and family counseling to patients • Collaborate with physicians, nurses, and interdisciplinary teams to support patient care • Connect patients and families with community resources and support services • Document patient progress and maintain accurate, confidential records • Support crisis intervention and provide guidance in high-stress situations • Advocate for patients' needs and assist in care planning and discharge coordination Requirements • D egree in Social Work (MSW) from an accredited program • Current state license as a Licensed Clinical Social Worker (LCSW) • 1+ year of clinical experience in a healthcare or mental health setting (preferred) • Strong counseling, assessment, and case management skills • Excellent communication and interpersonal abilities • BLS certification may be required depending on setting
