Employment Type: Full-Time About Us Care Resources is a community-based program for people 55 or older that promotes healthy, independent living and helps prevent nursing home placement. Work with the participants in this amazing program at our main facility in Kentwood with some home visits each week. Plus, enjoy a consistent weekday schedule with no weekend commitment for a great work-life balance! What You'll Do Perform intake screenings and make appropriate recommendations for services. Function as a key member of the interdisciplinary team, collaborate on care planning and service allocation decisions. Complete participant evaluations as needed and develop plans of care. Provide treatment to participants and implement physical therapy procedures consistent in achieving identified goals.
