Home Infusion Pharmacist - Full Time San Antonio, TX | On-Site $62-$68 per hour We are hiring a Full-Time Home Infusion Pharmacist to join a growing healthcare organization in San Antonio, TX. The Home Infusion Pharmacist will be responsible for dispensing medications, ensuring prescription accuracy, and providing patient education. This role works closely with healthcare professionals to support safe and effective medication therapy while maintaining compliance with all regulatory standards. Key Responsibilities • Dispense prescription medications and provide patient counseling • Review and interpret prescriptions for accuracy and clinical appropriateness • Monitor medication therapy for potential drug interactions or adverse effects • Collaborate with physicians, nurses, and clinical staff • Maintain complete and accurate patient medication records • Ensure compliance with federal, state, and pharmacy regulations • Support medication therapy management and patient education initiatives Qualifications & Experience • PharmD required • Active pharmacist license (or ability to obtain) • Experience in home infusion, acute care, critical care, or utilization review preferred • Strong knowledge of pharmacology, anatomy, and physiology • Pharmacy residency experience is a plus • Familiarity with aseptic technique and medication administration Skills • Strong communication and interpersonal skills • High attention to detail and accuracy • Ability to work effectively in a multidisciplinary healthcare team • Experience with pharmacy software systems • Understanding of insurance billing and reimbursement processes Compensation & Benefits • $62-$68 per hour (Full-Time) • Health insurance • Paid time off (PTO) • Employee discount • Referral program
