Job Description The Physical Therapist roles are located at University Crossing and Bartram Skilled Nursing Facilities in Jacksonville, FL! Under general supervision, participates in the evaluation, planning, recording, and implementation of patient care programs designed to maintain, improve, or restore physical function and to assist patients in reaching their maximum performance level. Manages a specific caseload of patients from admission through discharge.
