Overview Join our team as a Care Transitions Coordinator (Clinical Liaison), where you will play a vital role in guiding patients through the complexities of post-acute care. Our focus is on enhancing patient outcomes while ensuring referral sources experience exceptional satisfaction with our services. Responsibilities Develop patient-centered transition plans by integrating evidence-based clinical and preventative guidelines, fostering high-quality and efficient post-acute care delivery. Act as a representative for our home health branches, forging strategic relationships with health systems, hospitals, inpatient facilities, physicians, and executives. Qualifications Essential: Graduate of an accredited nursing, therapy, or social work program. Current license to practice in your respective state. A minimum of two (2) years of field experience. Knowledge of federal, state, and local regulations governing Medicare-certified home health and hospice operations. Proficiency with technology, including mobile devices. Preferred: Registered Nurse (RN) or Physical Therapist (PT) license is preferred. At least three (3) years of relevant field experience. Experience in home health or healthcare sales is advantageous. Requirements: Valid driver's license. Appropriate automobile liability insurance. Reliable transportation in good condition. Ability to drive safely in various weather conditions.
