Primary Care Physician - Mission-Driven, Underserved Community FocusLocation: Los Angeles, CASchedule: Monday-Thursday, 7:30 AM - 4:40 PM (Onsite) | Friday Remote (Administrative Day)Compensation: $222,445 - $370,741 + Comprehensive BenefitsPractice With Purpose. This is an opportunity to provide comprehensive, high-quality care to some of Los Angeles' most vulnerable and underserved populations - including individuals experiencing homelessness, mental illness, substance use disorders, and chronic medical complexity.You will serve as both a frontline clinician and clinical leader within a mission-driven, community-based outpatient environment focused on accessibility, justice, excellence, unity, and kindness. This role combines meaningful direct patient care with leadership responsibility and quality oversight.About the RoleThis physician provides full-scope primary and preventive care while supporting organizational clinical excellence. You will:Clinical Responsibilities Serve as primary care provider within a patient-centered medical home model Manage acute and chronic illnesses in a high-need population Provide care to patients with co-occurring medical, psychiatric, and substance use conditions Perform in-office procedures (I&D, arthrocentesis, nail removal, skin biopsy, cryotherapy, etc.) Maintain a productivity standard of 22 patients per day Complete and lock documentation within 48 hours Review and sign labs, imaging, and consult notes promptly Coordinate specialist referrals and e-consults Participate in outreach health screenings (TB, diabetes, hypertension, etc.) If working with adult populations, you may be asked to complete waiver training to provide Medication Assisted Treatment (MAT).Leadership & Administrative Responsibilities Assist the Chief Medical Officer (CMO) in clinical operations oversight Lead Provider Meetings when CMO is unavailable Support performance monitoring (HEDIS, STAR, UDS benchmarks) via chart audits and reporting Clinically supervise Advanced Practice Providers (APPs) Review minimum 50 charts for new providers Provide ongoing clinical oversight Conjoint supervision of medical assistants to improve care team efficiency Participate in Continuous Quality Improvement initiatives (PDSA methodology) Periodic on-call coverage (1 week approximately every 10 weeks) Occasional Sunday support shifts as needed Why This Role MattersThis is not traditional outpatient primary care. You will work with patients who often: Lack stable housing Have untreated or undertreated chronic disease Experience severe mental illness Struggle with substance use disorders Face systemic barriers to care Your work directly improves health equity, access to care, and whole-person healing in a community that deeply needs consistent, compassionate providers.QualificationsRequired: Active California Medical License BC/BE in Internal Medicine or Family Medicine DEA License BLS (CPR) Certification Ability to maintain Medicare, Medicaid, and managed care credentialing Preferred: Bilingual Spanish Experience in FQHC or safety-net setting Comfort treating complex, high-acuity outpatient populations Work EnvironmentThis is a mission-centered, community-based setting serving high-need populations.
