Position: Hospice Admissions Nurse (RN) Location: Columbus, OH Schedule: Full-time, M-F 8:30-4:30pm with every other weekend On-Call Rotation Travel: Required -350 Gas Card Provided + Auto Allowance + mileage reimbursement About the Role: We are seeking a compassionate and skilled Hospice RN Case Manager to provide patient-centered end-of-life care in homes, assisted living facilities, and skilled nursing settings. This role focuses on symptom management, emotional support, and ensuring comfort for patients and their families. As a key member of the interdisciplinary care team, you will coordinate services that uphold the highest quality of hospice care. Key Responsibilities: Conduct comprehensive patient and family assessments to develop individualized care plans. Monitor patient conditions, manage symptoms, and provide direct nursing care in home and facility settings. Administer medications and treatments in alignment with physician-directed plans of care. Collaborate with an interdisciplinary team, including physicians, social workers, chaplains, and aides, to provide holistic care. Educate families and caregivers on symptom management, medication administration, and end-of-life care practices. Maintain thorough and timely documentation of patient status and interventions. Communicate effectively with physicians and care teams regarding changes in patient conditions. Participate in on-call rotations to ensure 24/7 access to hospice support. Qualifications: Registered Nurse (RN) with an active license in the state. Minimum of one (1) year of experience in hospice, home health, or a related field preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills for working with patients, families, and care teams. Ability to travel locally within a designated radius.
