We are seeking a Neonatal Intensive Care Integrated Pharmacist for the day shift. Minimum Qualifications: Education: Graduate of an accredited School of Pharmacy. Licenses/Certifications: Must be a Licensed Registered Pharmacist in Texas; Pharmacy Sterile Product certification is required if you will be compounding sterile products. Job Responsibilities: Fill and label prescriptions, offering drug-related information and education to in-patients, out-patients, hospital staff, and medical teams. Dispense controlled substances while maintaining accurate inventory records. Supervise pharmacy technical staff and mentor or train pharmacy students or residents. Participate in the Medication Use Evaluation Program and Process Improvement Program. Maintain medication profiles and conduct reviews for any drug-related issues. Schedule and prepare I.V. solutions and additives. Comply with federal and state pharmacy laws as well as hospital policies and procedures. Initiate reports of medication errors or adverse drug interactions. Assist with inventory control by ensuring out-of-stock items are obtained when necessary. Engage in clinical interventions, including medication profile review, IV to oral conversion, renal dosing adjustments, antimicrobial streamlining, patient counseling, medication history retrieval, vancomycin/aminoglycoside dosing, therapeutic drug monitoring, and participation in multidisciplinary rounds. Document all clinical interventions. Ensure patient and staff safety, adhering to all Memorial Hermann policies and standards in budgetary management. Promote professional growth by engaging in mandatory continuing education and supporting departmental goals for organizational success. Serve as a preceptor and mentor to junior staff. Demonstrate a commitment to compassionate care for all community members, providing personalized experiences. Uphold Memorial Hermann's service standards by delivering safe, caring, and efficient experiences. Perform other duties as assigned. This position is scheduled for day shifts from 7:30 AM to 4:00 PM or 8:00 AM to 4:30 PM at Children's Memorial Hermann Hospital, an esteemed establishment recognized for excellence across various pediatric subspecialties, including Neonatology.
