Position Overview: We are seeking a dedicated Field Case Manager who will engage directly with injured workers and medical providers in a face-to-face environment to facilitate effective medical treatment and timely return-to-work plans. This role requires residing in the Grand Rapids, MI area as consistent travel will be involved, although you will manage your responsibilities from a home-based office. Qualifications: The ideal candidate will have at least 1.5 years of experience as a Field Case Manager specializing in workers' compensation. Core Responsibilities: Act as an advocate for injured workers, guiding and coordinating their care while they navigate the recovery journey. Empower injured workers to regain independence through effective communication, education, and resource identification. Identify suitable providers and facilities throughout the care continuum, ensuring that resources are utilized efficiently and effectively for optimal outcomes. Education and Licensing: Preferred qualifications include RN licensure or a bachelor's degree in health or human services, with certifications such as CCM, CDMS, or CRC.
