JOB DESCRIPTION DETAILS Job Summary : Licensed Practical Nurse at CalvertHealth will work with the value-based care staff and clinical staff to identify, schedule, coordinate and perform Annual Wellness Visits for CalvertHealth’s Centers for Medicare & Medicaid Services. Education : Completion of an accredited LPN Program and LPN - State Licensure Registration/Certification/Licensure : Current Maryland State LPN License in good standing CPR, BLS Certification NRP Certification within 6 months Experience : 1 - 2 years of experience as an LPN preferred Experience in primary care, geriatrics, or chronic disease management preferred Familiarity with Medicare wellness visit guidelines and documentation requirements Other Requirements : Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements. Proficiency with EHR systems and clinical documentation Strong communication and patient engagement skills Attention to detail and ability to follow clinical protocols The Licensed Practical Nurse (LPN) contributes to the provision of high-quality, cost-effective, healthcare as a provider of direct and indirect patient care.
