All exceptions are at the discretion of the Program Director. Completed PVCHC APRN Primary Care Fellowship Application Must be graduates (by orientation date -- with less than 750 hours of supervised prescriptive practice, as defined by Colorado DORA) or transitioning FNPs who demonstrate an intent to practice as a primary care provider Must have at least 2 years of RN work experience Certification in Primary Care specialty Active & Unrestricted Colorado RN & APRN license (eligible and complete by orientation date) Three (3) letters of recommendation At least one (1) from a clinic preceptor At least one (1) from a work supervisor No peer references will be accepted. Transitioning Nurse Practitioners will be considered on an individual basis Ability to commit to 12-month, full-time program Submit a 500-word essay addressing each of the following topics and submit by email to : Describe your work history and how it has prepared you to be a family nurse practitioner. Share a specific example (professional, academic, or personal) of a time you assisted people from an underserved population. What are your professional values as a nurse practitioner? Discuss your expectations and goals of the Fellowship. Explain how you will contribute to the teaching and learning environment of Fellowship and why you are an ideal candidate for this position.
